The Population Health Department at Union Health is looking for an Ambulatory Care Clinical Pharmacist for the Terre Haute campus. This role plays an integral role by managing the use of medications in hospital with goals include the selection, prescription, procuration, delivery, administration, and review of medications to optimize patient outcomes and assures that services are provided within professional standards and organizational policies to assist patients who lean on the Pharmacy team for answers.

Ambulatory Care Clinical Pharmacist Essential Job Duties:
Education, Training & Experience:
Advanced Pharmacy Degree (Pharm.D.) Board Certification preferred in the first year of eligibility. One to two years experience in ASHP accredited residency or equivalent experience preferred. Ambulatory experience preferred. Licensed or eligible for licensure in the State of Indiana. Must take licensure examination at the earliest convenience with licensure received within 75 days following first day of employment in position. Job duties of a graduate pharmacist may be performed under the supervision of a pharmacist. Failure to pass the licensure exam results in a change of status to Pharmacy Technician/Pharmacy Intern.
