Ambulatory Clinical Pharmacist
Overview Expected pay for this position is $59.90 - $70.47/hour. Actual pay will be determined by experience, skills and internal equity. POSITION SUMMARY: Reviews prescriptions, obtains clarifications, analyzes drug therapy, and regularly consults with medical staff, nursing personnel and others in order to coordinate delivery of high-quality pharmaceutical services to OSF HealthCare patients and/or participants. Responsible for compounding and dispensing medications according to established procedures and professional pharmacy practice. Coordinates medication therapy management and medication adherence programs to optimize medication outcomes in specified patient and/or participant populations. Collaborates with patient and/or participants care teams and providers through interdisciplinary rounding and consultative referrals. Demonstrates the knowledge and skills necessary to provide patient and/or participants care that is appropriate to the ages of those served. Qualifications REQUIRED QUALIFICATIONS: Education: Doctorate in Pharmacy (PharmD) Experience: 2 years of experience in hospital and/or retail pharmacy OR the equivalent experience gained through completion of a Pharmacy Practice PGY1 residency. Licensure/ Certification: License (or eligibility for licensure) to practice pharmacy in the state of Illinois and/or Michigan, as applicable to the location of work. Other Skills/ Knowledge: Interpersonal skills necessary in order to communicate effectively with physicians, patients/participants, and various other OSF HealthCare personnel when explaining medication use and gathering and exchanging other medication information. Analytical skills necessary in order to determine drug appropriateness and evaluate drug interactions. Demonstrated skills in independent problem solving, decision making abilities, and prioritization/delegation. Demonstrated knowledge of the principles of growth and development as well as the physical, emotional, and psychosocial needs of neonates, pediatrics, adults, and geriatrics. Excellent computer skills and the ability to process detailed information in an accurate manner. Competency in the use of an electronic medical record system. Compliance with state and federal pharmacy rules and regulations.
