This role of **Ambulatory Clinical Pharmacist** , with Banner Staffing Services is embedded in a primary care clinic: In-person and/or telehealth direct patient care, managing medications and chronic diseases. You will have collaborative practice agreements in place for anticoagulation, diabetes, hypertension, dyslipidemia, obesity, osteoporosis, tobacco cessation. You may also provide comprehensive med review and general patient education, including but not limited to medication therapy and lifestyle modifications. **The primary location is, 21772 S Ellsworth Loop Rd, Queen Creek, AZ, 85142, and possibly other Banner Medical Group Clinics (Chandler, Mesa, Phoenix, Peoria). Hours are 7AM - 3:30PM or 8AM - 4:30PM.**

**Registry/Per Diem positions do not have guaranteed hours and no medical benefits package is offered.** Completion of post-offer Occupational Health physical assessment, drug screen and background check (includes; employment, criminal and education) is required.
POSITION SUMMARY
This position actively participates on the healthcare team as a pharmacotherapy expert in the area of ambulatory care to provide high quality direct-patient care and serve as a resource and drug information specialist to all other healthcare staff. Contributes to the optimal selection of drug therapy and design and may implement patient-specific medication regimens to maximize therapeutic benefit, minimize toxicity and attain quality measure gap closure. Accurately and completely documents patient assessment data and clinical interventions. May provide comprehensive patient education to improve medication compliance and promote patient self-management skills. Must possess clinical acumen and skills necessary to assess and interpret data reflective of the patient’s clinical condition. Conducts educational activities, applies principles of performance improvement, and may participate in Committee activities as appropriate assisting with the development and implementation of evidence-based medication guidelines/protocols. May oversee residents, interns, pharmacy technicians and support staff as applicable, providing direction in completion of daily responsibilities.
CORE FUNCTIONS
1. Uses best practices and knowledge of internal or external business challenges to suggest improvements to services, processes or products. May provide oversight and assist with the development and implementation of evidence-based collaborative practice protocols. May participate in Ambulatory Pharmacy and Therapeutic (P&T) or other system committee activities as appropriate. Evaluates and recommends medications for formulary. Prepares formulary evaluations for P&T consideration.
2. Solves complex problems. Takes a new perspective using existing solutions. Performs medication therapy and disease state management, which includes critically evaluating disease states and adjusting medication therapy, per the ambulatory pharmacy collaborative practice protocols, to optimize patient regimens and improve clinical outcomes. Ensures utilization of appropriate and cost-effective medication therapy. Areas of focus may include, but are not limited to, disease state management (e.g. thromboembolic disease, diabetes, cardiovascular disease, COPD), medication management, adverse drug event prevention and management, and evaluating and addressing quality measure therapy gaps.
3. Interprets customer needs, assesses requirements and identifies solutions to non-standard requests. Interacts primarily with department and cross-department peers, supervisor, customers, peers’ managers, patients and physicians. Functions as a therapeutic consultant in areas that may include rational and appropriate use of medications, clinical pharmacology, pharmacokinetics, and disease state management. Demonstrates accurate and thorough documentation skills. Facilitates pharmacist-to-provider communication and clinical interventions.
4. Makes decisions within approved operating plans and objectives and within functional policies and precedents. Determines how and when to achieve results.
5. Participates in ordering or prescribing, administering and monitoring of all medications utilized. Internal customers may include Patients, Nurses, Licensed Independent Practitioners, Administrators and other health care team members for the purpose of integrating services, improving patient care and ensuring effective communication systems. External customers may include local, state, and national pharmacy organizations, State Board of Pharmacy, colleagues and peers, educational institutions and pharmaceutical representatives.
MINIMUM QUALIFICATIONS
A degree in Pharmacy from an accredited College of Pharmacy.
Completion of an accredited PGY1 pharmacy residency program or three years of previous clinical pharmacy experience in the ambulatory care setting.
Requires current and unrestricted pharmacy licensure in the applicable state of practice within 90 days of hire date. A current and unrestricted Pharmacist license (not intern) is required at the time of hire for BSS applicants.
Incumbents in Arizona are also required to provide the AZ Board of Pharmacy Wallet Card at time of hire.
Maintains good organizational, problem solving, and computer skills. Possesses good written and verbal communication skills for interfacing with all levels of staff, physicians, patients, and other contacts.
PREFERRED QUALIFICATIONS
Additional related education and/or experience, including Board Certification in a related field, preferred or PGY2 pharmacy residency program in ambulatory care.
