Under the direction of the Manager, Retail and Specialty Pharmacy Services, the pharmacist provides oversight and support for the development and advancement of specialty pharmacy services with a focus on providing clinical services to patients with specific disease or associated disease states per Collaborative Drug Therapy Management agreements, optimization of clinical workflow, identification and coordination of staff competency needs, and development of quality metrics for specialty pharmacy services.\n

Roles and Responsibilities:\n\nParticipates in Collaborative Drug Therapy Management (CDTM) agreements with providers to optimize patient outcomes.\nDesign and implement specific disease state protocols and medication order panels for the patient populations managed by specialty pharmacy services in collaboration with other pharmacy and non -pharmacy providers.\nCoordinate the identification, communication and resolution of problems relating to daily activities of specialty clinical pharmacist.\nIdentify need for specialty staff education and competency. Coordinate with department leadership in the planning and execution of specialty staff development programs, and competency assessment.\nAssist specialty pharmacy leadership in the development and maintenance of productivity and quality metrics and utilize this data to continuously monitor current state and recommend process improvements as necessary.\nServes as the clinical liaison between specialty pharmacy and ambulatory sites.\nCoordinate the provision of additional patient clinical data or published peer reviewed articles as necessary to facilitate the prior authorization process through various insurers\nAssist in preparation for URAC accreditation and ensure continuous compliance with accreditation standards.\nEnsure compliance with REMS programs.\nServe as a preceptor for pharmacy students and residents.\n\n

Required Skills and Experience:\n\nEducation - Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree.\nExperience - Completion of an ASHP accredited PGY1 Pharmacy Residency, or a Board of Pharmacy Specialties Certification is required.\nCompletion of a PGY2 Oncology Pharmacy Residency, or comparable work experience, and specialty pharmacy experience is preferred. Three years of experience in a clinical practice setting with strong working knowledge of pharmacy operations and clinical services is required. Certified Specialty Pharmacist (CSP) from the Specialty Pharmacy Certification Board preferred.\nSpecial skills - This individual must demonstrate strong leadership, verbal and written communication, presentation, organizational and data analysis skills. Capable of assuming responsibility for evaluating and monitoring drug therapy relative to the total needs\/problems of the ambulatory care patient in collaboration with physicians, nurses and other healthcare professionals. Must have strong process and project management skills.\nAccountability - This individual reports to Manager, Retail and Specialty Pharmacy Services and is held accountable to maintaining the highest of professional standards and supporting the vision, mission, and goals of the organization. Will be accountable through matrix reporting to the Manager of Specialty Pharmacy Services. Must complete all hospital and department competencies and requirements for employment and maintain annual continuing education credits as mandated by the State of New York Board of Pharmacy.\nComplexity - In personal and job-related decisions and actions, consistently demonstrates the values of integrity (doing the right thing), patient-centered (putting patients and families first), respect (valuing all people and embracing all differences), accountability (being responsible and taking action), and compassion (being empathetic).\nThis individual must demonstrate the ability to identify, plan, implement, and monitor programs and initiatives. Must apply new ideas, knowledge and skill gained from experience and education to current situations. Should possess thorough knowledge of current standards of pharmacy practice. This individual should possess exceptional time management and organizational skills.\nLicensure - Currently licensed in the State of New York. Must be certified immunizer in New York.\n\n

Physical Requirements:\n\nRepetitive use of hands and fingers (e.g., use of computer keyboard); may require lifting and carrying light loads (up to 40 lbs.), including boxes, equipment, and IV solutions and stooping and kneeling (e.g., to pick up items from the floor, to remove and replace items on lower shelves, and to file documents in lower file drawers); sitting, walking, or standing for long periods of time (4-8 hours) is often necessary; must be able physically to operate the equipment used for the job.\n\n

Pay Range Disclosure\n\n$115.398.40 to 145,600.00 per year\n\n
