Ambulatory Medical Social Worker

In this role, you will play a vital part in enhancing the lives of individuals, families, and communities in need. You will collaborate closely with clients to assess their needs, develop personalized care plans, and provide the support and guidance necessary as they navigate complex medical, social, and emotional challenges. SCHEDULE AND PAY This is a full-time (40 hour) position. Clinic Hours are Monday – Thursday 8AM - 7PM, Friday 8AM – 5PM. Scheduling needs will be discussed during interview. Salary range: $55,000-$60,000, depending on certification and experience. DUTIES + Responsible for providing patient care that addresses social determinants of health (SDoH). + Conducts psychosocial assessments of patients that address SDoH. + Develops individualized care plans based on patient needs and SDoH. + Counsels patients on options and provide emotional support to patients and families. + Facilitates transition of care planning and continuity of care for patients. + Organize and lead support and education groups for patients and families. + Serves as the main point of contact for patients who present with any abuse or neglect. + Assists with applying for medication, transportation, housing, and other assistance programs that help to address SDoH, + Responds to referrals within three business days. + Responsible for providing case management to referred patients. + Works collaboratively with the clinic community health worker. + Collaborates with the healthcare teams to ensure comprehensive patient care. + Educates patients and families about healthcare and community options and resources. + Conducts outreach advocacy and networking as necessary. + Advocates for patients' needs and rights within the healthcare system. + Assists patients in navigating the healthcare system and accessing resources. + Collaborates with community organizations to connect patients with additional services. + Assists patients with financial concerns and connects them to relevant resources. + Provides crisis intervention and emergency support. + Assists patients with phone calls related to obtaining resources. + Develops, maintains, and updates All Care Health Center’s Community Resource List. + Maintains documentation for all consultation notes, significant incident occurrences, phone conversations, etc. + Document patient interactions within 24 hours of contact and maintain accurate records. + Maintains documentation consistent with applicable regulatory requirements. + Other duties as assigned. MINIMUM REQUIRED QUALIFICATIONS + Bachelor's degree in Social Work from a CSWE-accredited program or an international program deemed equivalent by the CSWE. + Licensed as LBSW in Iowa or license eligible within 1 year. + One year of experience providing generalist micro and mezzo social work services. + Have a working knowledge of county/state benefit programs, the health care delivery system, local health care providers, and community resources and be able to help patients navigate these systems. + Strong social work assessment and care planning skills. + Excellent written and verbal communication skills. + Empathy and commitment to patient advocacy. + Crisis intervention skills. + Ability to handle sensitive information with confidentiality, professionalism, and compassion. PREFERRED QUALIFICATIONS + Experience providing medical social work services in an ambulatory or acute care setting. + Bilingual or multilingual. + Experience using an electronic health record. + Knowledge of medical terminology and the healthcare system.
