MAJOR RESPONSIBILITIES
The Population Health Virtual Pharmacy Team performs a variety of programs and services using population health management strategies and virtual communication to care for patients across the Advocate Health Midwest Region. These include chronic disease management using collaborative practice agreements to adjust and monitor medications, medication therapy management referrals, medication adherence outreach to patients with chronic diseases, and opioid weaning support. It is an ever-evolving environment, where we engage patients in self-management of their chronic diseases and collaborate with other healthcare professionals to improve health outcomes.
The Ambulatory Pharmacist – Population Health will focus on medication therapy management referrals, transitions of care, and supporting value-based care metrics such as medication adherence outreach to patients with chronic diseases, via telephone and/or video visits.
The ambulatory pharmacist would be joining a team of pharmacists and pharmacy technicians with varying pharmacy backgrounds
MINIMUM EDUCATION AND EXPERIENCE REQUIRED
License/Registration/Certification Required: Pharmacist license issued by the state in which the teammate practices.
Education Required: Bachelor’s Degree in Pharmacy
