Pay: $16,502 per year stipend (required # of hours: 1,700/year)

Day-To-Day

Develops trusting relationships with clients by providing support and advocacy to help achieve health goals Conveys the purpose and services of Community Health to the member population and the impact that program or service.  Meets member in clinic, facility, office, or at home to help identify social determinants of health impacting member's health and general well-being  Follow-up with health management/care plans with both members and providers Coach member in effective management of their chronic health conditions and self-care. 

Qualifications

High School diploma or equivalent

Any combination of 1 years health/social services experience and/or education 

Prefer candidates who live within a 10 mile radius of zip codes 75208 & 75211 Valid Texas Driver’s License and ability to be insured.
