Under the direction of the Annual Wellness Supervisor, the Annual Wellness Coordinator performs duties related to the scheduling, promotion of Medicare Annual Wellness Interviews and assist with printing HCC face sheets for RHPA Primary Care practices. The Annual Wellness Coordinator works closely with patients, Annual Wellness Specialists, Annual Wellness Specialist Lead, Annual Wellness Supervisor, CDI Director, Call-Center employees and RHPA Primary Care offices.

Overview of Responsibilities

1. Schedule all Medicare Annual Wellness interviews for Annual Wellness
Specialists and APPs in RHPA Primary Care offices.
2. Follow procedures set per RHPA Primary Care Practices and CMS for
scheduling Annual Wellness Interviews.
a. Utilize WPS portal to ensure scheduled visits are within timeframe for
billing purposes.
3. Utilize scripting for calling patients; including patients who refuse Annual
Wellness Interview.
a. Develop formalized process for patients who refuse Annual Wellness
Interviews and provide information to RHPA Quality Coordinator.
4. Coordinate schedules with Annual Wellness Supervisor to ensure Epic schedaligns with Annual Wellness Specialist monthly schedule.
5. Troubleshoots potential service gaps with schedules. Communicates gaps withAnnual Wellness Specialist, Annual Wellness Supervisor and primary care
supervisors to meet patients and office needs.
6. Provide ongoing education and training to front office staff and primary care
supervisors regarding Epic scheduling and WPS navigation.
7. Review schedule for ACO patients and provide HCC Facesheets for providers toensure documentation of chronic conditions.

8. Answers incoming telephone calls, quickly, professionally and with a pleasantone of voice.
9. Attends Wellness Health Fairs and other community events as assigned to
promote Annual Wellness Interviews.
10. Proficiency in Outlook, Teams, Smartsheet, Word and Excel.
11. Works independently, with minimal supervision, in daily routine.
12. Required to check email daily and communicate effectively with Annual Wellnteam members.
13. Demonstrates strong communication skills.
a. Articulates the objectives and benefits of community-based patient care.

b. Demonstrates sound presentation skills for Wellness Health Fairs.

c. Maintains collaborative working relationships with medical staff, nursing,
and other disciplines involved in patient care throughout the continuum.
14. Other duties as assigned, requested and/or necessary.

Education/Experience

Education Required: High School graduate or GED equivalent


Education Preferred: Associates Degree


Experience Required: Six months previous clerical experience with a general understanding of Medicare, previous work experience in a provider office or medical related position and knowledge of medical terminology.


Experience Preferred: Experience working Medicare ACO members

Schedule Details Day Shift. 32 hours weekly.
Monday-Thursday 8-430pm
