The anti-trafficking Case Manager position is funded through a partnership with the City of Philadelphia Community Behavioral Health Department. The anti-trafficking Case Manager will provide comprehensive, wrap around case management services for women experiencing behavioral health needs and coordinate services such as: therapy withdrawal management, rehabilitation, housing, identification, financial assistance, and ongoing monitoring with clients in the community. The New Day Case Manager will manage a caseload of 20 clients at any given point in time. The population to be served are individuals who have been victimized by commercial sexual exploitation, sex trafficking and severe forms of trauma.
ResponsibilitiesDuties:
Manage a direct service case load of 20 clients at any given point in time, both at the New Day Drop-in Center and in the community as a mobile case manager. Case Management duties including:Conduct initial screening, intake, and assessment of clients in need of case management services at the New Day Drop-in Center.Conduct biopsychosocial assessment of clients enrolled in case management.Coordinate withdrawal management and rehabilitation services for individuals with substance use disorders.Develop mutually agreed upon goal plans with clients addressing areas such as employment, education, vocational training, withdrawal management, housing, mental health, physical health, family, and financial goals.Ensure timely coordination of referrals to resources and the meeting of immediate needs upon each client’s enrollment into case management services.Develop individualized, client-centered, trauma-informed, and culturally sensitive service plans for self-sufficiency with women from the New Day Drop-in Center.Support clients in reintegration into the community to develop a sense of voice and regain power and control after suffering forms of severe trauma and sexual exploitation.Engage clients in life skills development activities to include budgeting, time management, emotion management, coping skill development, and others based on individualized service plan as a part of ongoing case management.Keep up to date case notes and ensure client confidentiality in all forms of communication.Administrative Duties to include:Reviewing and screening all new referrals for case management.Assisting with the assigning of new cases to case managers within the drop-in center.Gathering and documenting eligibility information for case management clients and submitting requests for new authorization numbers to Program Manager and as needed to CBH.Schedule and coordinate case reviews, data entry workdays, and other administrative efforts of the CBH case managers.Assist the Program Manager with stats and reporting, including monthly review of claims data.Participate in management of a 24/7 hotline for response to victims of HT and coordinate services needed for all potential victims.Submit timely reports on case management duties into Service Point, and in identified CBH reporting tools and other internal and external reports as necessary.Establish and maintain connections with community resources.Performs related duties as assigned. QualificationsRequirements: BSW or related degree required. MSW preferred. 2-5 years of experience working in human trafficking victim services or a related field (ex. Domestic violence, sexual assault, immigrant rights, workers’ rights) in a role that involves direct service provision. Experience with trauma informed care, preferably a working knowledge of Sanctuary Model for Trauma Informed services. Must have an understanding of the issue of domestic human trafficking. Experience working with substance use disorders. Must have reliable transportation to travel from site to site as may be needed in the performance of duties. Professional-level communications skills. Bilingual in Spanish preferred. As a Mandated Reporter, must provide proof of having completed training on recognizing and reporting child abuse meeting the training standards of PA Act 153 of 2014 within 30 days of employment with recurrent training required every five years thereafter. Meets the Child Protection Clearances required by PA Act 153 to include PA Child Abuse History Clearance, PA State Police Criminal Record Check and FBI Criminal Record Check. Must possess an appreciation for and understanding of the mission of The Salvation Army.
