Position Overview:

The Licensed Clinical Social Worker (LCSW) provides crisis intervention, information, referrals, and discharge planning for patients and their families, ranging in age from infancy to geriatrics. Serves as an expert and liaison to community resources. Provides coordination of complex discharge planning needs and referrals to appropriate support services. Achieves a timely, effective, coordinated discharge plan by involving other members of the health care team. Works closely with Clinical Reviewers to achieve the highest quality care and service and the best possible outcomes in a cost-effective manner. Evaluates patients for 5150 holds as appropriate. Facilitates Group Therapy sessions as needed.

Job Description:

JOB ACCOUNTABILITIES

Evaluates all referrals within one working day (24 hours).

Initiates and maintains a therapeutic relationship with patient and families.

Performs psychosocial assessment and treatment planning for patient and families

Performs Discharge Planning assessments and facilitates placement of patients with complicated medical and psychosocial needs.

Refers patients/families to appropriate care, resource and community programs

Applies principles and techniques of interviewing, counseling and crisis intervention to communicate effectively with patients/families, healthcare team members and community providers.

Refers issues around death and dying to the Chaplaincy Program.

Works closely with Clinical Reviewers to monitor acute care criteria ( InterQual)

Notify Risk Manager of possible risk issues.

Maintains accurate, current legible records on patient charts of all assessment, plans, interventions patient/family involvement, outside agency communications, interdisciplinary contacts and case closure.

Maintains a current file of community resources, and provides linkage between patient/family and services.

Records and submits accurate, timely productivity and other statistical data.

Must be available for scheduled coverage and flexing of work hours, and call as needed.

Evaluates patients for 5150 holds as needed and arranges transport to appropriate facility

Facilitates group therapy sessions as needed.


EDUCATION
Equivalent experience will be accepted in lieu of the required degree or diploma.

Master's: Masters Degree from a program accredited by the Council on Social Work Education

Other: 5150 training


CERTIFICATION & LICENSURE

LCSW-Licensed Clinical Social Worker


PREFERRED EXPERIENCE AS TYPICALLY ACQUIRED IN:

Two years experience in a Health Care setting and one year acute care hospital experience.

Previous experience in Discharge Planning preferred.


SKILLS AND KNOWLEDGE

Strong clinical psychosocial skills with ability to conceptualize clinical situations and demonstrate application of theory appropriate to specific age groups.

Knowledge and understanding of individual, child and family development, human behavior and communication, discharge planning and the influence of culture and values in social environments.

Ability to plan and implement an effective discharge plan for patients and families.

Knowledge of Medicare , MediCal , State and Federal regulations.

Knowledge of child, elder, domestic and dependent abuse reporting requirements.

Knowledge of Community resources and hospital contracts.

Knowledge of Hospital Policy and Procedures.

Computer skills, including Microsoft Office applications.

Good interpersonal and communications skills.

Ability to work under the supervision of the Supervisor of Social Services.

Strong clinical assessment and critical thinking skills.

Time management skills.

5150 Training.

Ability to facilitate Group Therapy sessions.

Pay Range is $47.38 to $63.68 / hour

The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs.
