General Summary: An exempt position responsible for providing immediate care services, including assessing, diagnosing, prescribing, treating, and educating patients. Works closely with physicians and other clinicians in a team approach to patient care.

 

Clinical Knowledge

Demonstrates knowledge of etiologies, risk factors, pathophysiology, presentation and epidemiology of medical conditions. Performs history and physical assessments of patients that are accurate, complete, concise, relevant and appropriate for age, gender and clinical problem. Orders appropriate diagnostic studies and correctly interprets the results. Utilizes critical thinking to synthesize and analyze collected data. Develops, implements, evaluates and alters the plan of care as needed. Manages general medical and surgical conditions based on knowledge of the indications, contraindications, side effects, interactions and adverse reactions of pharmacologic agents and other relevant treatment modalities. Counsels and educates patients and families. Demonstrates proficiency in procedural skills pertinent to practice area. Maintains accurate, complete, concise and timely documentation in the electronic medical record.

Procedures

Perform routine clinical procedures independently as outlined in practice agreements/scope of practice including, but not limited to, the following:

• Laceration repair with sutures, staples, or Liqui band

Splinting fractures Foreign body removal Incision and drainage Suture Removal Pelvic Exams Wart Removal

 

Patient Care

Provides care that is patient and family centered, compassionate, appropriate and effective for the promotion of health, prevention of illness and treatment of disease throughout the lifespan. Outpatient Specific Responsibilities (100%)

Conducts comprehensive health history and physical assessment of patients with acute health problems. Performs history and physical, develops, manages, and modifies treatment plans. Participates in daily huddle and collaborates with multidisciplinary health care team as needed to adjust plans of care. Facilitates referrals to appropriate community health facilities, agencies, and resources Discharges patients and educates patient on the instructions they provide the patient in the after -visit summary

 

Education: Successful completion of advanced practitioner training and/or certification as a practitioner (general or specialized) from an approved advanced practice provider program.

 

Experience: Minimum three years of advanced practice provider experience in a clinic setting, plus one year of advanced practice provider experience (preferred, but not required).

 

Other Requirements: Current state registered nurse license and/or Current state license as an advanced registered nurse practitioner or certified physician assistant with prescriptive authority. Current CPR certification.

 

Equipment Operated: Standard medical examination room equipment and instruments. May include use of computer/health information management system.

 

Work Environment: Combination of exam rooms and medical offices. Frequent exposure to communicable diseases, toxic substances, ionizing radiation, medicinal preparations, and other conditions common to a clinic environment.

 

Mental/Physical Requirements: Requires standing and walking for extensive periods of time. Occasionally requires lifting and carrying items weighing up to 10 pounds. Requires correct vision and hearing to normal range. Requires working under stress in emergency situations and occasionally may involve irregular working hours.
