Sentara Medical Group has a full-time opening for an experienced APP (NP or PA) to join our Ambulatory Remote Patient Monitoring team in providing primary care.

This is a new program in partnership with HealthSnap that is focused on preventive measures to help patients manage chronic conditions such as high blood pressure, diabetes, etc. with a goal of preventing admissions and readmissions to the emergency department.

Minimum Requirements:

Active NP or PA Must have experience practicing as an APP (3 years preferred) managing own patient panel and the ability to work independentlyTelehealth experience is a plusApplicants may live in the following states: Indiana, Nevada, South Carolina, South Dakota, Tennessee, Texas, Virginia, West Virginia, or Wisconsin
