ResponsibilitiesJob Description
Monitor the health status of an individual with disabilities or illness, and address their health-related needs, such as changing bandages, dressing wounds, or administering medication.Work is performed under the direction of offsite or intermittent onsite licensed nursing staff.Provide assistance with routine healthcare tasks or activities of daily living, such as feeding, bathing, toileting, or ambulation.May also help with tasks such as preparing meals, doing light housekeeping, and doing laundry depending on the patient's abilities.Upon completion of the program, the apprentice will be prepared to move into a Direct Support Professional Lead or Supervisory role and will be able to serve as a Journey Worker.Description of Work Process
Prepare and maintain records and case files, including documentation, such as clients' personal and eligibility information, services provided, narratives of client contacts, or relevant correspondence.Confer with clients to discuss their options and goals so that plans for accessing needed services can be developed, encouraging self-help activities.Ensure client safety and safe environment.Performing house management functions such light housekeeping, laundry, bed making and cleaning.Planning meals, shopping for groceries, preparing and serving food/meals, feeding, and cleaning up.Transporting/accompanying clients to appointments, running errands with or for clients.Reminding or assisting with self-administration of medication.Assisting with daily living skills such as toileting, bathing, and dressing.Reporting change in client’s conditions to supervisor.Ability to mentor and supervise Direct Support Professionals.Supervisory responsibilities included QualificationsIndiana DSP Lead Apprenticeship Criteria for initial enrollment:
• Must complete an application for the program
• Must be at least 18 years of age
• Must have a HS Diploma/GED
• Must be employed FT or PT (PRN is not eligible)
• Must be employed for 90 days or more (application can be completed prior to 90 days of employment, but applicant will not be enrolled in the program until after 90 days of employment)
• Cannot be on a Final Written Warning
• Job Performance – a review of job performance, including but not limited to prior Corrective Actions, will be conducted should we have more applicants than available spots.
Indiana DSP Lead Apprenticeship Criteria for continued enrollment:
• Must continue to meet all the initial enrollment criteria
• Must not be placed on a Final Written Warning during the duration of the program
• Must not have any more than 3 unexcused absences in the current review month (verified by Supervisor monthly)
• Must stay current on all the training required for the program
