OVERVIEW: Assess applicants in the community for eligibility and enrollment in our Managed Long Term Care program. Provide an overview of program benefits to applicant, family member(s), and care giver(s). Complete clinical assessments of applicant, review Plan information, complete enrollment documentation and develop initial plan of care. Complete periodic reassessments for members within our geographic area. RESPONSIBILITIES: + Conduct home and/or nursing home visit(s) to educate applicant, family and/or applicant’s caregiver(s) on program benefits and design. + Identify applicant’s health needs and complete all indicated assessments of applicant including but not limited to the UAS-NY and paraprofessional time tasking tool. + Utilize iCircle Services designated criteria along with clinical knowledge and expertise to develop the initial plan of care. + Initiate direct communication with health care providers involved in the care of the applicant to obtain complete and accurate information needed to complete service plan. + Collect information concerning eligibility, provider status, benefit coverage, and coordination of benefits. + Input visit and assessment data into required systems. Provide all forms to the Enrollment Coordinator or designee. + Complete all enrollment paperwork during home visit including but not limited to: enrollment agreement/attestation, medical release, HIPAA release, and any other documents required within agency timeframes. + Review the member handbook provider directory, services covered, etc. during in home visit. Provides copies of all required documents to potential member. + Completes periodic reassessments and develops updated plan of care. + Adhere to all HIPAA standards and confidentiality requirements. + Maintain accurate scheduling information at all times. + Act as community liaison to the Plan. + Perform all other duties as assigned by supervisor ESSENTIAL ACADEMIC AND EXPERIENCE REQUIREMENTS: + NYS RN license + Bachelor’s degree in Nursing preferred with 3+ years’ experience, preferably in home health and/or geriatrics + Experience in completing the UAS-NY assessments in a home care/managed care setting, preferred + Current case management experience preferred + Public speaking and/or health education experience preferred + Must be at least 18 years of age + Reliable transportation (valid NYS Drivers’ Licensed required). Ability to travel within assigned territory + Ability to work independently and motivate others + Ability to communicate effectively, both orally and in writing + Ability to lift at least 50 lbs. dead weight + Maintain all required certifications/training by State regulations and iCircle Services policy The listed salary range represents the organization's good faith and reasonable estimate of the range of possible compensation at the time of posting. The offered salary will be determined by: Applicant qualifications and experience, education, position specific licensing/training and departmental budgets.
