POSITION SUMMARY: The Assistant Community Care Manager will provide support to Community Manager (s) with planning, directing and organizing the delivery of LIfeStyle Options service sin a manner that ensures quality standards.

ESSENTIAL DUTIES:

Care coordination for residents in retirement communityMonitor Care plans to ensure resident needs continue to be met. Coordinates client weekly billing and transmittalConducts client evaluationsFill-in clients services as needed. Conducts caregiver orientation/training, as neededCoordination of client and caregiver schedules, approval of timeslips and entering corrections/changes in the scheduling software.Caregiver coaching and counseling as requiredPerforms the duties of the Community Manager in their absence. Other duties may be assigned

Maintains positive internal and external customer service relationshipsMaintains open lines of communicationPlans and organizes work effectively and ensures its completionMeets all productivity requirementsDemonstrates team behavior and promotes a team-oriented environmentActively participates in continuous quality improvementRepresents the organization professionally at all times

POSITION REQUIREMENTS & COMPETENCIES:

Certified Nurse’s Aide in good standing with the state is preferredTwo (2) years health care experience preferred.Ability to work independentlyMust have a valid driver’s license and reliable vehicle
