JOB SUMMARY Thames Valley Council for Community Action (TVCCA) is the lead contractor on a home visiting initiative funded through the Connecticut Office of Early Childhood. TVCCA has partnered with Day Kimball Healthcare, Lawrence + Memorial Hospital, Madonna Place, Visiting Nurse Association of Southeastern CT, William W. Backus Hospital & Windham Hospital to form the Eastern Connecticut Home Visiting Alliance. The Alliance will provide a system of coordinated referral and intake to serve families in Eastern CT with well-established, evidence-based home visiting programs. Effective and collaborative coordinated intake provides a single point of entry for access and referral of families to needed services. The Assistant Director will conduct a brief screen to identify needs and strengths as well as the information necessary for referral. This position serves a critical role in connecting with high-risk perinatal families and families with children under age 2 to home visiting services and support.
