Assistant Director, Social Drivers of Health Program

As the Assistant Director for the UCLA Health Social Drivers of Health Program, you’ll help leadership shape the program’s direction and drive evidence-based screenings and interventions. By using data-driven insights to inform decision-making, you’ll spearhead innovation within the program. You’ll manage a Community-Based Organization network by establishing strong relationships with community partners, coordinating outreach efforts, and overseeing the integration of community resources into the program’s initiatives. The role also involves enhancing efficiency within the program’s projects. You will: * Collaborate with the Director and Medical Director to establish program priorities and advance UCLA Health’s efforts to address social drivers of health and promote health equity. * Oversee the day-to-day operation of the program, coordinate cross-functional team, and work closely with other departments. * Collaborate with subject matter experts, healthcare professionals, and community partners to design evidence-based interventions that address identified social needs and risks. * Establish key performance indicators for the program and individual projects, tracking progress against these metrics and benchmarks. * Contribute to the development of evaluation frameworks to assess the impact of social drivers of health interventions and identify areas for enhancement. * Develop a community network of community-based organizations and agencies to support patients’ social needs. * Ensure the program’s adherence to and reporting of relevant standards, regulations, and reporting requirements to accrediting bodies, health plans, and government agencies. * Foster a positive, inclusive work environment that promotes collaboration, creativity, and accountability. Salary Range: $78,800 – $175,000 Annually
