Responsible for the direct supervision, training and support of assigned case managers, as well as carries their own caseload. Understands and effectively executes around managed care and all regulatory and funding requirements. Assists case managers in discharge planning and transition efforts. Pursues funding approvals in conjunction with Case Managers as needed, e.g., aging out clients. Oversees assigned case managers at Woods. Provides ongoing support, supervision and guidance with their case management responsibilities. In coordination with the Director, is responsible for training and development of training materials for Case Managers. Materials to include manuals and computer based training modules and platforms such as Relias. Topics are to include but not limited to customer service, meeting management, time management, case planning and coordination, discharge and transition planning and understanding managed care. Reviews and revises, training materials, as necessary. Provides Case Managers with a complete understanding of applicable states funding and social service streams. Ensures that all program-planning efforts comply with applicable accreditation standards, licensing regulations and funding agency requirements. Serves as a resource and assists Case Managers on compliance with regulatory compliance issues. Assists in the development of criteria and monitoring program evaluation systems, which address staff training and supervision, health/safety requirements and the tracking and training of all documentation required for the individuals on caseloads. Reports results up with recommendations for improvement, if needed. Responsible for the assurance that all paperwork is submitted according to deadlines and meets the internal and external requirements. Efficient in the use of a computer for professional correspondence and reports, including creating and tracking data and reports using electronic health record systems. Provides Case Managers with an understanding of individual benefits available to the clients and where applicable, assists case managers and families with procurement of such benefits, e.g., SSI, Medicaid. Monitors and ensures the continuation of individual benefits. Ensures and assists, as necessary, with the pursuance and confirmation of funding approvals for aging out clients, transfers and clients with special staffing needs in conjunction with case managers. Participates in Woods’ Admissions’ Committee, as needed. Completes and communicates all pre-admission paperwork and information for new admissions to Woods as well as aging out information for the education graduates. Assists in the tracking of all graduates the year prior to their graduation and keeps the programs informed with aging out information. Ensures efficient transition plans are in place to ensure smooth transitions and all necessary pre and new admission paperwork is completed timely. Provides oversight and technical assistance related to discharge planning, including sharing information and referrals to community resources, when necessary. Responsible to stay abreast of the newest developments in the field, specifically in relation to regulations and funding, and incorporates and implements changes, as appropriate. Completes all assigned trainings and assists in the development and updating of a manual for the training of new Case Managers to be used as a resource manual for current Case Managers. Assists in the development of internal quality assurance and evaluation systems for monitoring services to ensure consistency and satisfaction with each individual. Promotes visibility and engagement across programs for themselves and Case Managers. Maintains positive professional/ethical relationships with all employees/client parents/relatives/agencies etc. and facilitates effective working relationships with employees of other departments throughout the organization. Carries a smaller caseload of their own and responsible for associated responsibilities with that caseload. Also responsible to fill in for a Case Manager in times of absence or emergency. Works in coordination with other case management management team members and the records department to ensure system consistency and effectiveness, as needed. Chairs, participates in, or serves on various committees, as assigned. Assists in obtaining consents for various procedures/activities, as needed. Performs other duties as assigned to ensure the effectiveness and efficiency of case management services. Master’s Degree in Social Work or related field with three years of case management experience required. Supervisory experience in a facility providing services to the Developmentally Disabled or those with Traumatic Brain Injury preferred. Strong understanding of and prior experience working with managed care agencies preferred. Proficient with computers. Excellent communication and organizational skills, strong attention to detail. Valid driver’s license and reliable vehicle required. Must be able to read, write, enter dates and follow verbal/written directions, prepare written reports and complete written forms in English. Must be able to verbally communicate in English and direct clients, respond to clients’ needs. Must be able to lift a minimum of 50 pounds, be agile, physically pursue clients on foot, physically restrain (with appropriate training), bend, squat, reach, push up and stand for several hours at a time. Prior to employment, medical certification and child abuse clearances are required.
