FlexStaff is seeking an Assistant Director of Clinical Services to work for one of our external clients, a non-profit organization that provides home healthcare services to the elderly, disabled and chronically ill in the New York metropolitan area. The office is located in Amityville, NY.
Must have a NYS RN License. Additionally, an ideal candidate would have a nursing degree, and completed NYSED-approved infection control coursework; minimum of two to three (2 – 3) years of Clinical Staff Supervisory experience, as well as experience with CHHA, preferred.
This is a permanent, full-time job. Schedule: Monday – Friday 8:30 AM – 5:30 PM, 40 hours per week.
In this role you will be responsible for supervising all day-to-day operations of the Interdisciplinary teams for the LHCSA along with coordination of the multi-disciplinary integrated care in the community, Day Health Center, Diagnostic & Treatment Center.
Responsibilities include, but not limited to:
Provide oversight and support to site-based managers and clinical staff, including various departments like rehab, nutrition, social work, and home services. Lead Care Planning meetings, ensure timely follow-ups, and mediate care treatment disagreements. Allocate resources and ensure adequate team coverage. Develop and implement personalized care plans to improve participant satisfaction and outcomes. Oversee service provision affecting participant care and ensure timely care plan completion. Implement training and directives for team development. Collect and review operational data to ensure quality and cost-effective participant care. Participate in quality assurance and compliance activities. Collaborate with clinical departments to enhance care delivery processes. Address and resolve customer issues and grievances in coordination with other departments. Ensure complete and accurate participant documentation. Conduct timely Level 2 investigations. Review participant charts and address deficiencies. Participate in special projects and adhere to scope and competencies set by the PACE organization.*Additional Salary Detail
