Shift Worked: M-F, 9a-5p with flexibility Weekly Hours: 37.5 FLSA Status: Exempt Pay range: $60,000 - $65,000 The employee in this position will work closely with The Director of Day Services, Finance, nursing, Human Resources Department, other team members, and the individuals/families served to ensure that services are provided as identified and specified in the Habilitation Plans. As part of a team, an employee in this position is responsible for assisting individuals to successfully live independently or at home, participate in the local community, and promote the individuals’ integration into the community. POSITION DUTIES In a manner consistent with the agency mission and applicable laws and regulations, an employee in this position shall: + Ensure progress toward achieving the values of The Day Habilitation Purpose Statement: “Encouraging people to explore, discover and realize their full potential by fostering connections through shared interests while celebrating what makes us unique,” as well as values of the HCBS Medicaid Waiver, i.e., Independence, Integration into the Community, Individuality, and Productivity. Ensure documentation reflects progress toward achieving higher levels of these values. + In line with CPU’s commitment to CARF Accreditation, the employee will demonstrate a commitment to continually enhance the quality of services and programs with a focus on the satisfaction of the persons served. + Assist in developing service designs and proposals that enhance existing supports and develop new supports to achieve valued outcomes in the lives of the individuals we support. + Provide direct supervision to the Day Habilitation Coordinators, including consistent support, coaching and training, monthly group team meetings, and 1:1 supervisory support meetings. + Promote service development and evaluation, focusing on activities that build independence, autonomy, productivity, and choice as appropriate to the individual. + Supervise service development and evaluation, focusing on activities that promote independence, autonomy, productivity, and choice as appropriate to the individual. + Provide feedback to the Director of Day Services and the Director of Quality Improvement to assist in the agency’s response to statements of deficiencies resulting from internal and external audits of services. This includes providing constructive feedback and developing corrective action plans within areas of responsibility to ensure that those plans are implemented quickly and effectively. + Ensure that staffing supports are consistent with individual’s service plans. + Assist with interviewing, hiring, and onboarding staff. + Oversight of program personnel-related issues/systems to include payroll, PTO, performance evaluation, progressive discipline, and working with the Human Resources Department on continued training, recruitment, and retention. + Ensure compliance with all applicable state and federal regulations. + Ensure timely and correct completion of timesheets, Medicaid billing, and documentation for services provided. + Ensure that records are in compliance and meet standards consistent with funding accountability. + Oversee the implementation of programming and data collection for habilitation goals. + Responsible for record-keeping of staff training. + As needed face-to-face contact and phone calls with staff, individuals and families. + C onduct supervisory and team meetings with staff as needed. + Ensure staff remains in compliance with all annual mandatory training. + Ensure staff development through regular individual meetings, observations, and evaluations consistent with NADSP Core Competencies and the Agency Corporate Compliance Plan. + Participate on agency standing committees as assigned. + Review and approve timesheets for accuracy and approve time-off requests accordingly. + Intervene as needed to safeguard the individual and others during behavioral crises, including implementation of Strategies for Crisis Intervention and Prevention (“SCIP”) techniques as indicated. + Manage sensitive or crisis situations in a manner that is respectful and maintain the dignity of the individual. + Ensure the safety and well-being of the individuals. + Conduct, supervise, and participate in monthly fire drills. + Keep records of all fire drills. + Maintain a high level of cleanliness and the physical integrity of the site, conduct monthly physical plant maintenance walk through. + Ensure adequate staffing coverage. + Perform other appropriate job-related activities as required by your supervisor or as circumstances warrant. Supervisory Responsibilities This position directly supervises Day Hab Coordinators Essential Skills and Experience · Ability to physically assist individuals as required; and, to intervene physically when appropriate using approved techniques to safeguard the individual, oneself and others during behavioral crises. · Ability to relate appropriately to individuals, their families and their advocates; and, to constructively relate to and communicate with peers and supervisors and to be able to function successfully as part of a team. · Ability to work independently (when appropriate). · Ability to use sound judgment while following agency policies, practices and procedures. · Successfully complete all agency-required training and to keep required training status current on an ongoing basis. · Possess a valid driver license with a driving record acceptable to the agency’s insurance carrier and possess the ability to drive agency vehicles to transport individuals served to and from activities or events in the community at large. · Knowledge of community resources. · Possess computer proficiency, particularly with Microsoft Office. · Ability to effectively supervise and provide support to direct support professionals. Qualifications + Bachelor’s Degree in Sociology, Psychology, or related field and a minimum of 2 years of supervisory experience supporting individuals who have intellectual and developmental disabilities in an environment regulated by NYS OPWDD. + Excellent verbal and written communication skills + Proficiency in computer skills + Excellent organizational skills + The employee is required to successfully complete all required trainings Physical Demands and Work Environment The physical demands described below are representative of those that must be met by an employee to successfully perform the essential functions of this position. While performing the duties associated with this position, an employee (using approved agency lifting techniques) may be called upon to support and lift adult individuals with developmental and other disabilities; assist in and out of showers/on and off toilets; and, in and out of vans or other vehicles. An employee in this position must be able to physically intervene (when appropriate and in an approved manner) to protect the individual from causing harm to themselves or others and to safely evacuate the community/van/facility during an emergency. These duties may entail lifting/supporting/transferring up to 50-70 pounds on one or more occasions during the work shift and occasionally up to 200 or more pounds with assistance from other staff or devices. Employees will regularly be required to stand; walk; sit; use hands (or hands and arms) to carry, finger, handle or feel objects, tools or controls; reach with hands and arms; climb stairs; balance; stoop; crouch and kneel. Employees will also be regularly required to hear (with or without hearing devices), see (with or without corrective lenses) and speak.
