Duties include but are not limited to direct supervision of the care team as assigned, direct oversight of the care of residents, the evaluation of residents, development and maintenance of resident care plans, communication with medical professionals and families about the needs of residents, maintaining compliance with regulatory standards, and communicating with community and Support Center team. This role is essential in ensuring residents are treated with respect and dignity, have their individual needs recognized, and encourage independence.
Essential Functions
1. Works directly with the Director of Health & Wellness to ensure effective, efficient daily operations, and consistent quality
resident care and service delivery within the Care department.
2. Responsible for the daily management and oversight of the memory support area to include supervision of community-based care team, including contract agency staff when applicable, conducting periodic training and skill competency checks and effective provision of resident care.
3. Participates in care conferences with residents and family members to discuss resident needs and appropriate service delivery in association with service plan updates, as assigned.
4. Assists with the onboarding of Care Team members.
5. Works with team members to implement Company directed policy, procedure, and applications.
6. Collaborates with the Director of Health and Wellness to manage monthly team member schedules, arranges replacement
staffing, when necessary, to promote adequate staffing levels in support resident service plans, program delivery, and budget.
7. Assist in move-in and return process for new and returning residents.
8. Conducts evaluations and assessments of potential, new, and current residents, as directed.
9. Oversees the medication management program in partnership with the Director of Health & Wellness to include order
management, administration, storage, documentation, assessment of resident who self-administer medications, and as
mandated by the State regulations and guidelines and Company policy and procedure.
10. Performs ongoing observation of resident physical and psycho-social needs and coordinates with Care team to drive
quality and improvement initiatives.
11. Performs daily, weekly, and monthly safety and infection control checks of the community and grounds, assisting the
Executive Director and the Director of Health and Wellness for resident health and safety purposes.
12. Communicates with families regarding scheduled physician, dental, eye, and other medical appointments.
13. Assures in-house ancillary medical services such as podiatrist, doctor visits, psych visits, labs, x-ray, ambulance, etc. are
scheduled and followed through.
14. Participates in the maintenance of resident records including review of documentation performed by resident assistants,
monthly MAR reviews, setting up new resident and thinning records appropriately.
15. Requires assuming the role and responsibilities of a caregiver or medication technician, in order to meet resident needs and
staffing requirements.
16. Assures that all resident assistants / caregivers are familiar with the residents’ individual conditions and needs and are trained to provide the resident services in an effective, professional, and sensitive manner.
17. Collaborate with the Director of Health and Wellness to conduct team member coaching, performance reviews, and retention activities.
18. Works with the community leadership team to ensure team member completion of required training and education.
19. Maintains / monitors resident centered service plans for residents. Coordinates bathing and shower schedules in alignment
with resident service plans.
20. Collaborates with the Director of Health & Wellness to promote team member and resident safety and implement strategies
to reduce risk.
21. Responds in a timely manner to requests of residents, families, guests, vendors, referral sources, and outside contacts.
22. Supports DHW in maintaining compliance with local, state, and federal rules and regulations as applied to services
delivered in the specified care area. This includes areas such as staff selections, training and oversight, resident care and
documentation, and compliance with HIPAA, OSHA, Blood Borne Pathogen Guidelines, and any other rule that applies
to the safe, consistent operation of the community.
23. Supports community leadership team in identifying, developing, and implementing continuous quality improvement
opportunities such as Quality Assurance and Quality Assessment programs.
24. Fosters teamwork with co-workers, shows initiative in overcoming obstacles, takes actions which assist others beyond job
responsibilities, and has a positive impact on co-workers, residents, and visitors. Has working knowledge of the
organization.
25. Perform other duties as assigned or needed.
Educational Experience
• Licensed Practical Nurse
• Management experience and demonstrated ability to manage a team. Minimum of three years’ experience in assisted living
preferred.
Certifications, Licenses, and other Special Requirements
• Current CPR / First Aide Certification
• Preferred experience in the senior living environment, specifically dementia and Alzheimer’s care
• Experience with medication pass and certified as Med Tech as required by state law.
• In accordance with state law, may need to possess current state certification and follow regulations to maintain certification
currency.
• Must meet all health requirements, including TB.
• Must pass criminal background check.
• Minimum of 18 years old.
