Responsible for supervising an interdisciplinary team of clinicians in the field and maintaining operations. Collaborates with all members of the health care team with a focus on promoting evidenced based care within a healing culture for the patient population served and their families.


Principal Accountabilities:

1. Works with interdisciplinary team members to assess and establish plans of care. 
2. Collaborates with all members of the health care team with a focus on promoting evidence based care within a healing culture for the patient population served. 
3. Mentors field staff in all aspects of caring for patients.
4. Ensures current proficiency of knowledge regarding Medicare COPs, Epic, SHP, PBI, Medline. 
5. Perform chart reviews, annual performance appraisals, joint visits in field. 
6. Performs other related duties as assigned.
