Physician Affiliate Group of New York (PAGNY) is adding an Associate Medical Director to the group at NYC Health + Hospitals/Lincoln. The NYC Health + Hospitals Street Health Outreach and Wellness (SHOW) program utilizes mobile units to provide health screenings, vaccinations, wound care, basic material necessities to New Yorkers who are unsheltered. A further goal of the program is to connect patients to NYC Health + Hospitals facility-based clinics, where they can receive ongoing medical care. Lincoln is an acute care teaching hospital academically affiliated with Weill Medical College of Cornell University.

Responsibilities Serving unsheltered populationThis position will be partially based at facility-based Primary Care Safety Net Clinic and partially based out of mobile clinics as part of the Street Health Outreach and Wellness program
While staffing the mobile clinic, canvas surrounding neighborhood to engage with patients experiencing homelessness Offer limited medical services while encouraging patients to visit van or clinic for further evaluation and servicesParticipate in scholarly activities, including teaching medical students and/or residentsRobust analytic support from Office of Ambulatory Care & Population HealthAccess provided to NYC Health + Hospitals Quality Academy training for emerging leaders Qualifications Board Certification or Board Eligibility in Internal MedicineX-waiver for prescribing buprenorphine or willingness to obtain one
