Associate Patient Care Coordinator
Description We are offering a long-term contract employment opportunity in the healthcare industry for a Clinical Administrative Coordinator. Based in El Paso, Texas, this role is vital in providing administrative support, facilitating service coordination, and promoting optimal health through member services and community resources. Responsibilities: • Accurately process incoming requests via fax, email, and phone, documenting in the service coordination system as necessary. • Initiate daily member phone calls, identifying issues and referring to the service coordinator and other health plan departments as needed. • Schedule, cancel, and document new and/or follow-up appointments for service coordinators and other designated team members. • Evaluate initial incoming requests into the department, gather demographic and benefit data, and determine if the request needs to be referred to a registered nurse, social worker, or transition specialist. • Monitor portal to ensure timely completion of initial assessments and reassessments. • Manage the intake of MNLOC and ISP requests of identified members. • Document decisions into OICS and provide communication to the Service Coordinator regarding the outcome. • Assist internal and external departments by providing communication and education regarding case management and transition services and processes. • Provide ongoing education with members regarding managed care, benefits, and access to community resources Requirements • Proficient in Microsoft Office Suite including Excel, Word, and Outlook • Excellent communication skills, both written and verbal, in English and Spanish language • Proven customer service skills, with the ability to handle inbound and outbound calls professionally • Experience in documentation, with the ability to maintain logs and distribute faxes • Knowledge of insurance policies, Medicare, and health care sector • Ability to schedule appointments and coordinate various tasks efficiently • Experience in training staff and implementing company policies • Proficiency in using About Time and Consumer Electronics software • Understanding of benefit functions and progress reporting • Proficient in word processing and basic statistics • Ability to multitask and manage time effectively.
