As a Behavioral Care Manager you will assess patient needs, develop personalized care plans, and collaborate with healthcare providers to ensure comprehensive care. Behavioral Care Managers also work with patients and their families to provide education, support, and resources to promote overall well-being and improved health outcomes.

Key Responsibilities:

Clinical Responsibilities

Conduct assessments: Evaluate patients' behavioral health needs, including mental health and substance abuse concerns.Develop care plans: Create personalized plans to address patients' behavioral health needs, in collaboration with healthcare providers.Provide education and support: Educate patients and their families about behavioral health conditions, treatment options, and self-management techniques.

Care Coordination Responsibilities

Coordinate care: Collaborate with healthcare providers, social workers, and other stakeholders to ensure comprehensive care.Facilitate referrals: Connect patients with community resources, specialists, and support services as needed.Monitor progress: Track patients' progress, identify barriers to care, and adjust care plans accordingly.

Administrative Responsibilities

Document patient information: Maintain accurate and up-to-date records of patient interactions, care plans, and progress.Communicate with healthcare team: Share patient information and care plans with healthcare providers and other stakeholders.Participate in quality improvement: Contribute to quality improvement initiatives to enhance patient outcomes and care processes.

Other Responsibilities

Advocate for patients: Support patients' rights and interests, and help them navigate the healthcare system.Stay current with industry developments: Participate in ongoing education and training to stay up-to-date on best practices and industry developments.

Education and/or Experience Qualifications:

•             High School Education OR Two years of relatable employment history

Experience in Office Management (preferred)

Additional Qualifications:

•             A valid driver's license and a reliable vehicle.

•             Strong communication and interpersonal skills.

•             Ability to work independently and as part of a team.

•             Flexibility and adaptability in a fast-paced environment.

•             Compassion, empathy, and a genuine desire to help others.
