Remembrance, Resolution, Reconciliation, Restoration, Renewal About the Position: The Behavioral Health Care Manager (ECM) Inland Empire Campus (BH CM) for the Enhanced Care Management (ECM) - Community Supports (CS) program supports the program and Advocates by assuring the quality of clinical service is being maintained. Ensures that clinical records are accurate, current, and meet Medical Assistance standards and regulations. The Behavioral Health Care Manager provides expertise and guidance to improve student care, program effectiveness, and clinical outcomes.  Strong clinical background, experience in care management, and a passion for transforming service delivery through innovative practices. ECM is a comprehensive, patient-centered approach to healthcare that aims to improve outcomes for high-need populations.  This involves the coordination of medical, behavioral, and social services to address the full range of student needs, focusing on those with complex and chronic conditions.  ECM programs are designed to optimize care delivery, reduce unnecessary hospitalizations, and improve the overall quality of life for students. Full-Time Non-Exempt Salary Range: $69,500 Moreno Valley, CA. 92553 What You Will Do: + Supports ECM students with behavioral health conditions, with particular attention to ECM Students with SMI and/or SUD needs, through brief interventions and behavioral activation strategies. + Primarily works with a caseload of Students with behavioral health needs (majority of Tier 1 or 2). + Utilizes brief behavioral health therapeutic interventions as necessary to improve the student’s ability to manage their own health. + Provides brief crisis interventions and warm hand-offs to local crisis resources as needed to address behavioral health needs. + Provides Student and family education about chronic medical and behavioral health conditions to improve health literacy. + Provides formal and informal training and support for ECM Students on behavioral health conditions, including treatments and evidence-based treatments. + Assists with the coordination of medical and behavioral health access issues with PCP offices, specialists, and ancillary services. + Tracks medical and behavioral health outcome measures in a patient registry. + Ensures smooth transitions of care, coordination with psychiatric hospitals for M1 and with IEHP transitions of care team for M2, regarding client admission/discharges. + Collaborates on Student care issues with other ECM Care Team members, participating in weekly systematic case reviews and ad hoc case reviews. + Works with Students to identify health/wellness goals and incorporates these goals into the Student’s Care Plan to facilitate communication among Students and Providers. + Aids RN Care Manager in the overall review of student Comprehensive Health Needs Assessment upon completion by another care team member. + Engages with Students, both in-person and on the phone, utilizing evidence-based approaches (such as Motivational Interviewing) that promote collaboration between the student and their medical/behavioral team, as well as increase the student’s sense of control over their whole health. + Provides consultation for all students of the team when discussing tasks of a clinical nature. + Supporting the learning and practice of the Teaching and Therapeutic Community model as well as training faculty that incorporates essential elements of the Teaching and Therapeutic Community. + Attend workshops, meetings, and trainings as requested by supervisor. + Performs other related duties as assigned . What You Will Bring: Required: + Master’s degree in behavioral health, public health, or other human services related field. + Licensed or registered professional in good standing with California Board of Behavioral Sciences (BBS) as ASW, LCSW, LMFT, AMFT, LPCC, or APCC. + Minimum of 5 years clinical experience, with at least 2 years in care management, population health, or related field. + Expert knowledge of clinical practices, care coordination, and patient-centered care. + Strong analytical skills with the ability to interpret clinical data and translate findings into actionable strategies. + Willingness to travel locally as needed to support program initiatives, trainings, and stakeholder engagement. Preferred: + Previous Clinical experience working with individuals recently released from jail or prison a plus. Previous Supervisory experience. Certifications or Licenses: Required: + BBS licensure or registration as outlined above. + Valid Driver License Preferred: + N/A Skills/Abilities: + Excellent organizational skills and attention to detail. + Excellent written and oral communication skills. + Extensive knowledge of non-profit organizations. + Proficient in Microsoft Office Suite or similar software. + Knowledge of contracting process and associated local, state, federal and other regulations. + Ability to multi-task, identify problems, provide recommendations to management teams, and implement any applicable systems. + Ability to work constructively with diverse people and with parties that may have divergent perspectives and interests. + Understand and appropriately apply Amity policies and procedures and adhere to agency-wide practices and regulations.
