Behavioral Health Care Navigator *Remote*

The Behavioral Health Care Navigator is responsible for performing duties in support of the core functional areas of behavioral health within the Care Management and Utilization Management programs. In this position, you will be the front line responder to inbound calls for behavioral health (BH). 

 

Providence Health Plan welcomes 100% remote work for applicants who reside in one of the following States:

Oregon Washington: Clark County

This position is affiliated with Providence Health Plan and will be tied to a location and compensation range in Portland, Oregon

 

Required Qualifications: 

2 years of experience working with patients/members in a managed care or a facility based health care setting, preferably within a mental health related field 1 year of experience in behavioral health and social determinants of health navigation and care coordination 2 years of experience working in one of the following specialty areas, preferably within a mental health related field: Certified Nurse Assistant Certified Medical Assistant Certified Crisis Interventionist Health Care Advocate - Behavioral Health Customer Care Coordinator -Behavioral Health  Equivalent experience per manager discretion

Preferred Qualifications: 

Bachelor's Degree - Health Related Field Experience in crisis intervention, Trauma Informed Care, Motivational Interviewing Current Level Tech III or Clinical Support Coordinator Experience with Community Services, Safety Net Services, or Vulnerable Population programs
