As a Behavioral Health Case Manager, you'll coordinate care, provide direct services, and assist clients in accessing a range of community resources to support their individualized treatment plan goals. Key Responsibilities:

Provide all services within the organization's Code of Conduct and policiesDemonstrate understanding of health service systems and assist clients in accessing resourcesCoordinate care and participate in the development of the client's individualized treatment planConsult and cooperate with community resources to facilitate linkage, referral, and follow-upProvide direct services to adults, children, and families; coordinate family involvementHelp clients navigate financial entitlements and access healthcare coverageAssist clients in learning skills for personal health management and self-advocacyMonitor and assess client functioning and progress; update treatment teamDemonstrate understanding of mental illness and its treatmentEvaluate services to identify necessary improvements and increase outcomesProvide continuity of contact and coordinate access to housing, social services, and other resourcesUnderstand and respond to client rights; advocate on their behalfAssist clients in developing appropriate social skills and behaviorsServe as a link between clients and community services/resourcesPerform other duties as assigned

Education and/or Experience Qualifications:

Bachelor's degree in social service, education, behavioral health, or a related fieldDocumented experience in case management services preferredCertified as a Behavioral Health Case Manager II with the Oklahoma Department of Mental Health and Substance Abuse Services

Required License/Certification (one of the following):

Current certification as a Behavioral Health Case Manager II

Additional Qualifications:

Knowledge of case management and rehabilitation principles related to mental/behavioral healthUnderstanding of the challenges faced by individuals with mental illness and available community resourcesKnowledge of treatment modalities and legal requirements in mental/behavioral healthAbility to communicate ideas clearly and effectively, both verbally and in writing
