What You’ll Do

In this leadership role, you’ll be the driving force behind the success of your team and the care they deliver:

Lead & Inspire: Supervise and mentor a team of clinicians, empowering them to provide exceptional patient care.

Coordinate Care: Ensure smooth communication and collaboration between patients, families, and healthcare providers.

Develop Care Plans: Oversee the creation and execution of individualized care plans that address each patient’s unique needs.

Ensure Compliance: Maintain adherence to clinical guidelines, regulatory requirements, and company policies.

Drive Quality: Continuously assess and improve care delivery to achieve the best outcomes for patients.

What You’ll Bring

We’re looking for a leader who is passionate, organized, and ready to make a difference:

Registered Nurse (RN) with an active state license

2+ years of experience in home health or behavioral health care

1+ year of supervisory or leadership experience

Strong proficiency with clinical systems like Workday and HomeCare HomeBase, as well as MS Office

You will report to the Director Nursing or Clinical Administrator. This is not a full list of responsibilities. Salary for the Behavioral Health Clinical Team Manager ranges from $90,000 to $100,000, based on experience, with potential for additional earnings. Elara Caring is an equal opportunity employer.
