CARE COORDINATION TEAM (CCT) LEAD
The Care Coordination Team provides a single point of contact to assist individuals who are not effectively connected with the services and support they need to transition successfully from higher levels of care to effective community-based care. This includes services and support that affect a person’s overall well-being (such as primary physical and mental health care, housing, peer and natural supports, education, vocation, and the justice systems). Care Coordination is time-limited with a heavy concentration on educating and empowering the persons served until they are adequately connected to the care that meets their needs.
• This is an advanced professional position working as the Care Coordination Team (CCT) Lead, supervising a Case Manager and Peer Mentor.
• Provide weekly supervision to assigned staff.
• Participate in weekly staffings with the CCT and funder/Managing Entity (ME).
• Ensure that the CCT is adhering to the Evidenced Based Practice (EBP) Guidelines as defined by the ME
• Conduct in depth clinical assessment of high-risk population including issues of alcohol use and/or drug use, domestic violence, sexual abuse, sexual assault, child abuse and elder abuse.
• Ensure the timely completion of the Level of Care Utilization System (LOCUS) assessments for every phase of care.
• Serve as single point of accountability for the coordination of an individual’s care with all involved parties (i.e., criminal justice, childcare, primary care, behavioral health care, housing, vocation, etc.).
• Ensure that the CCT is engaging individuals in their current setting, such as the Crisis Stabilization Unit (CSU), State Mental Health Treatment Facilities (SMHTF), homeless shelter, detoxification unit, etc. Individuals served should not be expected to come to the CCT.
•Participate in and ensure timely completion/submission of care plans with the individuals based on shared decision-making that emphasizes self-management, recovery, and wellness. Care Plans must include transition to community-based services and/or supports.
• Ensure that the CCT is providing the frequency of contact as described in the CCT Guidance Document and Evidenced Based Practice (EBP)
• Ensure that the CCT is available to provide on-call support 24/7
• Coordinates treatment with community, governmental and service providers, across systems, to include behavioral and primary health care as well as other services and supports that impact the social determinants of health.
• Assists client with completing assistance applications to obtain benefits as applicable (i.e., Supplemental Security Income (SSI), Social Security Disability Insurance (SSDI), health insurance, Veterans Affairs benefits (VA), etc.)
• Ensure linkage to psychiatric services, if needed, within seven (7) days of discharge from higher levels of care. If no appointments are available, ensure documentation is in the medical record and notify the ME.
• Coordinate with the ME to identify service gaps and request purchase of needed services not available in the existing system of care.
• Develop partnerships and agreements with community partners (i.e., managed care organizations, criminal justice, community-based care organizations, housing providers, federally qualified health centers, etc.) to leverage resources and share data.
• Notify the ME of individuals identified for Care Coordination from the priority population as specified in the CCT Guidance Document
• Observes, evaluates, and monitors reports and records program participant status and progress.
• Maintains associated clinical records electronically using computer-based applications.
• Prepares accurate and timely written reports on assigned caseload using computer-based software applications, including quality and/or quantity of work completed.
