BEHAVIORAL HEALTH COUNSELOR

DESCRIPTION OF POSITION
The Behavioral Health Counselor provides consultation, education, and psychosocial services to patients and family members. Assesses psycho-social needs of patient and provides consultation to medical and other personnel regarding impact of psychosocial factors on patient illness. Discusses plan with multidisciplinary team to meet the psycho-social needs of patients and their families, helps enable patient to utilize medical care and access community resources. Role includes accessing medical records to perform ongoing performance improvement audits.
QUALIFICATIONS
Professional/Educational Graduate of an accredited college or university with a Master's Degree in psychology, social work, clinical/mental health counseling or related field. Two years of experience in a clinical setting related to Behavioral Health patients required. State of IL license as a LCSW or LCPC, or related field required within one year of hire date. DCFS Mandated Reporter Certificate of Completion required.
Personal Requires good oral and written skills and knowledge of community resource and services. Work requires excellent interpersonal skills and the ability to maintain composure in difficult situations. Able to manage multiple tasks in an organized manner; high degree of flexibility required. Self-directed learner; enthusiastically seeks ways to improve patient care and motivate others to participate in improvement activities. Possesses leadership skills and knowledge of hospital operations. Communicates effectively with physicians, patients and families, and all care providers. Possesses advanced conflict resolution skills. Ability to work independently and be self-directing.
