What you will do:
Provides behavioral health services as a member of the multidisciplinary team assisting with the monitoring of and providing direct support to the patients/residence in the facility within his/her appropriate scope of practice.
Assists with assessment, planning, and implementation of the approved plan of patient care which is provided through individual and group sessions.
Responsibilities:
Obtains and interprets information about the patient and his/her needs.
May conduct patient education sessions, as assigned.
Observes and records significant changes reporting them to appropriate person(s) immediately.
May conduct psychosocial evaluation and gather appropriate information to support a current DSM diagnosis as assigned.
Assists in screening and/or orientation of new patients to program and program expectations.
Develops treatment/service plan for assigned residents in a timely manner.
Assists with discharge planning.
Plans appropriate activities for assigned patients/residents with team.
Includes patient/resident, family, other health care professionals in formulation of plans.
Administers care that is consistent with established treatment/service plan.
Conducts patient education groups, as assigned.
May provide Group Therapy and/or didactic group sessions.
If required to supervise patient’s self-administration of medications, properly checks and documents activity (Community Residence).
Demonstrates ability to intervene and manage patient crisis according to policy.
Maintains open communication with referral source(s).
Contacts community agencies to whom patients are being referred in order to provide relevant information prior to discharge.
Provides other services to patients as needed.
What you will need:
HS diploma or GED. Associated degree preferred.
CASAC-T Required.
1 year experience working in an addiction or behavioral health related field.
Strong writing and communication skills are required.
Experience working with a diverse population and a strong understanding of multicultural issues is preferred.
Pay Range:$17.28 - $26.67
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
