Description

We're seeking a dedicated PRN Behavioral Health Crisis Case Manager to continue the growth of our Social Work Case Management team.

 

SHIFT: Afterhours (4p-12a, 12a-8a) and weekend all shifts. 

 

***Will need to be able to clock in from multiple locations; Will serve at any Emory Healthcare Emergency Room.

 

Client Assessment and Evaluation: Conduct thorough assessments to evaluate client's mental health status, behavioral issues, and psychosocial needs, utilizing standardized risk assessment tools and techniques to gather comprehensive information. 

 

Care and Treatment planning: Collaborate with patients, families, community support, and healthcare providers to develop individualized care plan based off assessment and tailored to both medical and psychosocial needs. 

 

Interdisciplinary collaboration and communication: Communicate and collaborate with those included in patient care team including but not limited to Physicians, unit nurses, social workers, security personal, and outside hospitals and agencies to coordinate and expedite the patient through the continuum of care management 

 

Crisis Intervention and Stabilization: Provide immediate crisis intervention, including de-escalation techniques, brief counseling, safety planning to help stabilize patients experiencing acute psychiatric crises, including but not limited to those with suicidal ideation, severe anxiety, psychosis, or substance induced issues. 

 

Patient and Family Education: Educate and provide guidance to patients and their families about mental health conditions, assessment and treatment process, and available community resources.

 

Discharge Planning and Referrals: Facilitate referrals and/or resources to appropriate inpatient of outpatient mental health services ensuring smooth transition of continued care to address individualized needs. 

 

Documentation: Maintain accurate and timely documentation of patient assessments, treatment plans, and outcomes in compliance with the hospital policies and regulatory standards in the electronic medical record (EMR) system. 

 

Advocacy: Advocate for patients needs and rights, ensuring patients are given culturally competent and respectful care within the healthcare system. 

 

Professional development and Education: Participate in professional development opportunities including workshops, seminars, and conferences to stay current with best practices and regulatory requirements, providing leadership and guidance of knowledge to other healthcare support team to ensure best patient care. 

 

Compliance and Safety: Adhere to ethical business practices including all hospital policies, procedures, safety protocols, infection control, and patient confidentiality in order to provide quality improvement initiatives and safe, therapeutic care.

MINIMUM QUALIFICATIONS:

Must have a Masters in Social Work, Psychology, Counseling, or related field from an accredited institution. 1-year recent experience in behavioral health setting preferred, 3 years recent healthcare experience preferred, experience in Acute Care setting preferred. LMSW or LAPC license in the state of Georgia required, or license eligible and license obtained within 90 days of hire. Must have working knowledge of software/Eemr applications. Must meet all quality and productivity expectations and successfully complete yearly competencies. Must be able to work 4 shifts per month.
