Behavioral Health Intake Representative

Department: Behavioral Health Admin

Shift: Days

Daily Work Times: 6:00 a.m. - 4:30 p.m.

Scheduled Biweekly Hours: 70

Starting Pay: $16.90 - $23.67

  Position Summary:  Ensures that requests for services and procedures are processed in an effective and efficient manner. Completes the intake process which may include coordination of services, data entry of referral information, general inquiries, and other referral and reimbursement issues. Provides a positive experience for the customer and maintain defined service standards.   Essential Functions and Responsibilities: 1. Coordinates agency referrals and inquiries, document and process all paperwork for patient care services, with accuracy and timeliness. 2. Provides accurate information regarding agency and community services that may meet client/family needs. 3. Ensures continuity of care for all clients admitted to the agency by coordinating with appropriate personnel including hospital discharge planners, physicians, client/families, staff and agencies to which clients are referred.            

Qualifications:

Required:

Associates degree from an accredited college or university.  OR High School diploma/GED and three-years experience working in health care or customer service
 

Preferred: 

Bachelors degree in human services, natural science, social science, marketing, health care administration, or business from an accredited college or university. Licensed Bachelor of Social Work (LBSW) in the State of Michigan. Experience working in hospital setting with utilization management, case management or behavioral health. .
