WELLBE INTRODUCTION

The WellBe care model is a Physician Led Advanced Practice clinician driven geriatric care (care of older adults) team focused on the care of the frail, poly-chronic, elderly Medicare Advantage patients.  This population is typically underserved and very challenged with access to care.   To address these problems, we have elected to bring the care to the patient, instead of trying to bring the patient to the care. Care is provided throughout the entire continuum of care – from chronic care and urgent care in the home, to hospital, to skilled nursing facility, to assisted living, to palliative care, to end of life care.  WellBe's physician/advanced practicing clinician led geriatric care teams’ partner with the patient’s primary care physician to provide concierge level geriatric medical care and social support in the home as well as delivering and coordinating across the entire care continuum.

Job Description

GENERAL SUMMARY

The Geriatric Psychiatrist supports the overall Social Services and Behavioral Health (SSBH) Department, participates with SSBH leadership on clinical program development and clinical educational needs for WellBe as a whole.  In addition, the geriatric psychiatrist will work with the entire WellBe team of clinicians through consultation, as well as individual patients as identified. The geriatric psychiatrist advises the primary care provider on appropriate medications and other treatment strategies, also providing the care team with educational support on diagnosing and managing behavioral health conditions.

Practices the WellBe mission: To help our patients lead healthier, meaningful lives by delivering the most Complete Care.Collaborates with a clinical “Community Team” consisting of an interdisciplinary team in managing the health and wellbeing of patients living in their homes.Has a successful relationship with the Market Regional Vice President, and Regional Medical Officer to achieve quality outcomes and growth in the community.Providing leadership, vision, and direction for the creation and progressive growth of the WellBe Behavioral Health (BH) programProviding medical leadership for the BH program by establishing the psychiatric program's standard of care.Acts as a Behavioral Health Subject matter expert to guide WellBe’s field team members.Communicating effectively with referring providers; keeps providers informed about recommendations for neuromodulation care, expected duration of services, interval updates, and outcomes.Collaborates and strategizes with the Director of Social Work to develop appropriate patient and employee directed behavioral health/ psychiatric health programming with a focus on psychotherapy to improve health outcomesImproves staff competence by providing training and discussions regarding the interrelationship between psychosocial and physiological problems, psychotropic medications, crisis intervention, and related issues.Develops clinical pathways based on common behavioral health conditions amongst patients (i.e Anxiety, depression, substance abuse, schizophrenia, bipolar disorder, etc)Collaborates with Director of Pharmacy to create the appropriate enterprise medication formularies.Helps patients by evaluating medical and psychosocial conditions, develops and implements treatment plans, including prescription of psychotropic medications.Evaluates patients and develops treatment plans by interviewing patient and family; conducting physical examinations; observing behaviors; reviewing medical history and related documents; selecting, administering, and interpreting psychological tests; and ordering laboratory tests and evaluating results from a cognitive, emotional, developmental, social, and behavioral disorders lens.Treats patients by utilizing psychotherapeutic methods and medications; discussing progress toward goals with patients; providing instructions; monitoring effect of medications; and supervising staff provision of services.Maintaining the quality of service and maintaining capacity to meet volume demands. Programmatic growth/expansion by identifying opportunities, educating referring providers, soliciting referrals, and recruiting other team members as needed.Understanding the impact on local and national landscape involving the WellBe clinical model consisting of: Health Policy, Socioeconomics, competition, CMS, regulatory, and other clinical data relevant to quality patient care.Prioritizes and acts on the most promising opportunities to improve outcomes/performance of the Community metrics (i.e. clinical, cost/revenue, partnerships, and quality measures).Spends time fostering key relationships in the Community NetworkPartners with WellBe Leadership to grow the organization, innovate the model and improve execution.Develops constructive Cross-functional / Matrix Relationships with WellBe business partners to enable the model.Other tasks needed to accomplish team’s objectives/goals.

Educational Requirements:

Doctor of Medicine (MD) or Doctor of Osteopathy (DO)Board Certification in PsychiatryCompletion of an ACGME-accredited General Psychiatry residency training programCurrent and valid state medical license requiredMinimum of 5 years of experience in psychiatry.Preferred: Completion of an ACGME-accredited Geriatric Psychiatry Fellowship training program

 

 Required Skills and Abilities:

Basic Life Support (BLS) certifiedHave a valid driver’s license, have access to a car and willingness to drive to patient homes/patient location as well as an active auto insurance policyEHR medical documentation experienceStrong computer skills, including Word, Excel, and PowerPointStrong verbal, written, presentation, and interpersonal communication skillsExcellent bedside manner and time management skills

 

Supervisory Responsibility: No supervisory responsibilities.

 

Travel requirements: Travel may be required up to 50% locally or nationally

 

Work Conditions: Ability to lift up to 20lbs.  Moving lifting or transferring of patients may involve lifting of up to 50lbs as well as assist with weights of more than 100lbs.
