I. Major Responsibilities:
1. Conducts initial evaluation of patient's physical condition including reviewing patient's medical history,
charts, x rays, and other relevant data. Obtains additional information from patient as needed. Analyzes
and interprets data and assesses individually current functional ability. Research specific problems as
required to make differential diagnosis.
2. Consults with physician regarding any contradictions or alternative forms of therapy. Assesses patient's
progress, as related to their rehabilitation potential Designs treatment plan based on initial evaluation and
assessment. Selects purposeful activities suited to the patient's emotional, intellectual, and physical
functional level to upgrade the patient's performance to maximal function.
3. Designs exercise programs for patient to perform between treatment sessions. Maintains all required
departmental records, i.e., progress notes, treatments, charges, etc. Reassesses patient's condition daily
and modifies goals according to patient's present status. Educates patients concerning diagnosis, proper
treatment, and preventative measures.
4. Establishes short- and long-term goals for patients. Applies occupational therapy skills such as the
application of modalities manual therapy activities, etc. Provides treatment in both individual and group
settings. Assesses patient's progress, as related to their rehabilitation potential. Forecast’s capabilities and
capacity of patient's future performance. Discusses patient's treatment and progress with physician
including any new or inconsistent findings. Ensures that Occupational Therapy continues throughout the
patient’s stay unless the patient refuses therapy.

5. Refers individual to various patient services, i.e., Physical Therapy, Speech Therapy, etc. as needed.
6. Consults with supervisors and advisors; and provides feedback to other patient care team members in
the most appropriate thera¬peutic program.
7. Contacts appropriate personnel regarding patient's discharge to home, nursing home or rehabilitation
center.
8. Educates patient, family and other caregivers in therapy program and goals. Participates in patient care
meetings, collaborates with team members, and assists with discharge planning. Plans, instructs, and
motivates patient/family to home exercise programs. May order equipment for patient and adjusts to suite
in¬dividual needs.
9. Collaborates with team members to assure patient coverage and equitable work distribution.
10. Evaluates patients suitable for treatment by registered physical therapists and physical therapy
assistants. Discusses appropriate program with therapist. Instructs therapist in any unfamiliar occupational
therapy procedures or techniques.
11. Creates an environment that supports continuous clinical and professional development of staff and
self.
12. Participates in quality and utilization review programs.
13. Provides input to Clinical Supervisor/Occupational Therapy with the design implementation and
evaluation of clinical education program and policies/procedures of occupational therapy area.

II. Position Qualifications:
License/Certification/Education:
Required:
1. Graduate of an accredited Occupational therapy program and current unrestricted license to practice
Occupational therapy from the Commonwealth of Massachusetts.
Experience/Skills:
Required:
1. Effective verbal and written communication skill; demonstrated advanced clinical knowledge and ability to
instruct / mentor others.
2. Excellent customer service skills and ability to work cooperatively with all members of the healthcare
team.
Unless certification, licensure or registration is required, an equivalent combination of education and
experience which provides proficiency in the areas of responsibility listed in this description may be
substituted for the above requirements.
Department-specific competencies, including age-specific competencies and their measurements, will be
developed and maintained in the individual departments. The competencies will be maintained and
attached to the departmental job description. Responsible managers will review competencies with position
incumbents.

III. Physical Demands and Environmental Conditions:
Work is considered standing and educating at a board while running groups. Sitting during meetings and
treatment teams. Pace of work is fast in an acute setting. Physical movement is required in exercise/yoga
groups. Position requires work indoors in a normal hospital/ office environment.
