Behavioral Health Patient Navigator

Summary:

The Behavioral Health Patient Navigator is a Registered Nurse for the out-patient Certified Community Behavioral Health Clinic (CCBHC) setting working with adults and children.

Patient navigation/care coordination refers to the assistance offered to clients to help them access and then chart a course through the healthcare system and overcome any barriers to quality care.

 

Responsibilities:

The role of the behavioral health RN is challenging and changes on a continuum with client behavior(s). The following framework is needed for new and tenured nurses to safely transition into behavioral health nursing practice.

Works collaboratively with the interprofessional team to ensure an evidence-based plan of behavioral health care that is coordinated with any required medical care.

Ensures compliance with CCBHC service requirements per policy/procedure provides primary care screening and monitoring for both adults and children to include building relationships with community resources and contacts (i.e. primary care providers (PCPs) dentists specialty providers other hospitals treatment centers and state agencies) and assisting patients to gain access/referrals to those services/resources; educating patients/caregivers to assist with understanding illness treatment and follow-up.

Ensures the team is kept appraised of a patient�s condition coordinates resource serves as a liaison between the psychiatry team community health agencies primary and specialty care providers recovery centers and staff to delivery high quality individualized care.

Documents patient�s progress compliance and other essential information in the medical record. Ensures patient�s care team primary care and/or sub-specialty providers are provided timely information on patient�s progress and additional needs.

Demonstrates skills for practicing as a nurse in the out-patient/community settings in relation to different modes of care within a given patient demographic:

Completes Nursing physical/behavioral health assessments and required physiological psychological and psychosocial and substance use screenings (CIWA/COWS) in-person and via scheduled telephone/virtual encounters.

Coordinates client care to ensure access to appropriate healthcare providers/home services while initiating and maintaining ongoing communication with patient/family as necessary.

Documents patient�s progress compliance and other essential information in the electronic medical record. Ensures patient�s care team is provided timely information on patient�s progress and additional needs.

Collects and reports information to ensure compliance with CCBHC quality standards expectations and reporting requirements if appropriate.

Reviews of intake questionnaires identifying/or connecting client to PCP services review of scheduled lab draws/values.

Identifies physical and psychosocial health-illness status of individuals history taking behavioral health assessment/biopsychosocial screening.

Professional and analytical skills in evaluating patients to plan and provide patient care and monitor the care management process and treatment plan for each patient

Reconciles orders/medications.

Provides initial nursing assessment for clients assigned to outpatient clinical team from referral source (outside hospitals/care organizations).

Identifies client need for assistance with managing medications/finances and escalating concern to appropriate party.

Adheres to Universal Precautions and Covid-19 precautions (donning/doffing PPE) as needed.

Safely prepares and administers IM injections.

Demonstrates cognitive appropriateness to practice: RN�s must know how and know when to perform tasks collaborate with internal/external healthcare providers to manage complex cases and long-term conditions.

Demonstrates (shows/does) development of practice and ability for self-reflection and evaluation.

Demonstrates knowledge of when to consult with Gateway Nursing Leadership and Risk management (Escalate Care)

Performs other duties as assigned.

Orientation to nursing behavioral health practice will be provided and is outcome and competency based. The orientation process allows experienced RNs with med-surg primary care and behavioral health experience to develop an autonomous and safe practice in the out-patient/community behavioral healthcare setting to meet the needs of the client base.

The RN must function the fullest extent of their licensure as defined by Lifespan�s Gateway Healthcare and the Rhode Island State Board of Nursing to meet the ever-changing needs of the clients that Gateway Healthcare serves.

The Registered Nurse will collaborate with multi-disciplinary team members in developing innovative programs to solidify relationships and associations that foster adoption and sustenance of innovations.

 

Other information:

EDUCATION:

ADN BSN or Diploma in Nursing are required.

Must have current licensure to practice as a Registered Nurse in the state of Rhode Island.

Licensure as Registered Nurse in the State of Rhode Island by the Rhode Island Board of Nursing or licensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing.

EXPERIENCE:

Two (2) years nursing experience in the med-surg addiction medicine critical primary or surgical care setting outpatient behavioral health experience preferred

Obtain certification in a behavioral health specialty within two years of hire matriculation toward BSN within 3rd year of hire.
