**Job Description:**
The Clinical Social Worker utilizes clinical expertise to perform psychosocial assessments, develop and implement plans of care appropriate care team, assess crisis situations to appropriately provide needed resources and or referrals for the patient, and to provide family and individual treatment as indicated. This position works collaboratively with patients, patients significant other, families, healthcare providers, insurers, community resources, and all other involved parties.
**Job Essentials and Responsibilities:**
+ Provides comprehensive intake examinations and behavioral health assessments, and determines accurate DSM-5/ICD-10 diagnoses based on clinical information.
+ Provides behavioral health services to individuals, couples, families and groups, using a variety of treatment approaches and tools.
+ Makes appropriate referrals for patients needing services outside the scope of the immediacy of primary care.
+ Maintains complete and timely clinical records on all clients.
+ Provides consultation to other staff members as requested and participates in staffing of cases.
+ Provides consultation and education services to individuals, agencies, institutions and groups as assigned.
+ Participates in staff training and attends staff meetings.
+ Responsible for the review of client files to assess dangerousness and violence evaluation of family violence referrals.
+ Responsible for making referrals to a variety of social services and medical resources as needed.
+ Participates in the screening process of new referrals to the department as needed.
+ Promotes mission, vision, and values of SCL Health, and abides by service behavior standards.
+ Performs other duties as assigned.
**Minimum Qualifications**
+ Masters degree in Counseling or Masters degree in Social Work is required.
+ Current Licensed Clinical Social Worker or Licensed Clinical Professional Counselor license
+ Current BLS certification or ability to obtain within 3 months of hire is required
+ Two (2) years clinical experience in related field is required
**This position requires hospital credentialing which takes 90 days - start date will align when credentials are granted.**
**Physical Requirements:**
Interact with others by effectively communicating, both orally and in writing.- and -Operate computers and other office equipment requiring the ability to move fingers and hands.- and -See and read computer monitors and documents.- and -Remain sitting or standing for long periods of time to perform work on a computer, telephone, or other equipment.- and -May require lifting and transporting objects and office supplies, bending, kneeling and reaching.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
$34.06 - $52.55
