Coverage Area: Fall River/New Bedford
Schedule: Full-Time, Afternoon/Evenings

As a Behavioral Health RN Case Manager, you’ll take the lead in:
✅ Assessing patient needs—physical, psychosocial, and environmental—to develop individualized care plans
✅ Managing a caseload, coordinating, and evaluating treatment plans
✅ Conducting initial evaluations and ongoing assessments during patient visits
✅ Communicating changes in patient conditions to the Clinical Supervisor, physicians, and care teams
✅ Educating patients, caregivers, and staff on behavioral health philosophy and symptom management
✅ Providing leadership and oversight to LPNs
✅ Offering compassionate support to patients and families

✔ Current, unrestricted RN license in the state
✔ 1+ years of nursing experience in a clinical care setting
✔ Willingness to travel within the assigned coverage area (50%)
✔ Reliable transportation, valid driver’s license, & auto insurance
✔ Associate or Bachelor’s Degree in Nursing (preferred)
✔ Experience in home care (preferred)
