Job Description:

Behavioral Health RN Case Manager - Full-Time, Salaried

Schedule: Monday to Friday, 8:00 AM - 5:00 PM

Key Responsibilities:

Patient Care Management: Oversee and coordinate a caseload of behavioral health patients, assessing their physical, psychosocial, and environmental needs.

Plan of Care Development: Create and implement individualized treatment plans, adjusting them as necessary.

Collaboration: Communicate significant findings to the Clinical Supervisor, physician, and other healthcare professionals to ensure optimal patient care.

Education & Support: Educate patients, families, and care staff on behavioral health best practices, symptom management, and follow-up care.

Supervision: Lead and supervise behavioral health aides and LPNs to ensure top-tier care delivery.

What You'll Need:

Current, unrestricted state RN licensee

1+ years of experience in a clinical care setting as a nurse

Able and willing to travel within branch/office coverage area.

Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.

Must be able and willing to travel 50%

Associates Degree or Bachelors Degree in Nursing is preferred

Previous Home Health or Hospice Experience is preferred

Previous experience with Homecare HomeBase is preferred
