Your Role as a Behavioral Health Clinical Team Manager


As a trusted leader, you’ll have the unique opportunity to enhance patient outcomes while mentoring a dynamic team. Here’s how you’ll make a difference:

Lead Exceptional Care Delivery: Oversee and coordinate home health services tailored to behavioral health needs. Manage patient and provider assignments, ensure compliance with regulations, and prioritize timely starts of care.

Champion Quality & Compliance: Approve and evaluate individualized care plans to maintain the highest clinical standards. Seamlessly integrate and coordinate physician orders.

Build a Culture of Compassion: Inspire and support a team of healthcare providers, ensuring patient needs are continuously assessed and met with excellence.

Streamline Operations: Manage processes for physician orders, staff communication, and interdisciplinary coordination for holistic care.

What You Bring to the Role

Education: Associate’s Degree in Nursing (Bachelor’s preferred).

Experience: At least 2 years as a Registered Nurse in Home Health or Behavioral Health, with 1+ year in a leadership role.

Expertise: Familiarity with enterprise systems (Workday, HomeCare HomeBase) and MS Office Suite.

Licensure: Current state RN license.
