Oversee patient care services, ensuring timely, diagnosis-specific home health services.Lead and coordinate care teams, patient assignments, and referrals to maintain optimal care delivery.Supervise the development, implementation, and updates of individualized patient care plans.Approve plans of care, evaluate proposed changes, and ensure alignment with clinical protocols.Facilitate communication among patients, families, physicians, and the care team.Ensure all regulatory guidelines are met and services are started promptly.Integrate input from all relevant healthcare providers into cohesive care plans.What You BringEducation: Associate's Degree in Nursing (required).Experience:2+ years of experience as a Registered Nurse in Home Health or Behavioral Health.1+ year in a supervisory role.Licensure: Current state RN license.Technical Skills:Proficiency with enterprise systems like Workday and HomeCare HomeBase.Strong knowledge of Microsoft Office Suite (Word, Excel, Outlook).Leadership OpportunityThis is your chance to lead a team and transform the delivery of behavioral health care.
