Meeting with patients to complete psychosocial assessments, treatment plans, as well as individual, group and family therapy as deemed appropriate for each individual patient. Conducting case management activities as well as discharge planning and maintains appropriate documentation.Providing appropriate therapeutic and educational interventions, services and is knowledgeable about community resources. Performing other duties as assigned.* This role requires a Master's prepared in the field of Clinical Social Work, Mental Health Counseling, or Marriage and Family Therapy, or other related field.

Required Certification/License:
BLS (Basic Life Support) Driver's License Driver's License LMFT (Marriage and Family), LMHC (Mental Health) or LCSW (Social Work) - PreferredRequired Education:

2 years clinical related experience or 9 month internship. - Required 1 year of relevant experience with Master's - Preferred
