Work hours: PRN (As Needed) Day Shift

Education

·     Master's Degree Counseling or Social Work required

Experience

·     2 years of healthcare related Counseling or Social Work preferred

Licenses and Certifications

·     LMSW - Licensed Master Social Worker upon hire required Or

·     LCSW - Licensed Clinical Social Worker upon hire required Or

·     LPC - Licensed Professional Counselor upon hire required Or

·     LPC-I - Licensed Professional Counselor Intern upon hire required Or

·     LMFT - Licensed Marriage and Family Therapist upon hire required Or

·     LMFT-A - Licensed Marriage and Family Therapist Associate upon hire required And

·     BCLS - Basic Cardiac Life Support prior to providing independent patient care and maintained quarterly required And

·     CPI - Crisis Prevention Intervention Training upon hire required And

·     ACPI - Advanced Crisis Prevention Intervention Training maintained annually 6 months required Or

·     CPI - Crisis Prevention Intervention Training 30 days required And

ACPI - Advanced Crisis Prevention Intervention Training maintained annually 12 months required

 

What You Will Do

·         Provides individual, family, and group psychotherapy and counseling utilizing modalities consistent with the facility philosophy.

·         Conducts unstructured individual, family, and group therapy as scheduled or ordered by physician to address goals identified on patient treatment plan.

·         Provides didactic group therapy as scheduled and assigned.

·         Adapts treatment to reflect age-specific, developmental and cultural considerations.

·         Demonstrates competence with adolescent treatment.

·         Demonstrates competency with cognitive behavioral therapy.

o    Effective individual, family, and group therapy provided in a timely manner meeting group therapy baseline competency elements

o    Documentation of therapies provided is clear, concise and reflective of required level of clinical expertise.

o    Patient goals for treatment and progress towards goals reflected in documentation.

o    Groups planned, delivered, and documented in a timely and effective manner meeting patients goals for treatment

o    Knowledge of CBT and other appropriate brief therapies and interventions evident

o    Knowledge of age-specific and cultural-specific dynamics demonstrated

·         Participates in the assessment of patients. Conducts psychosocial assessments to determine patients and familys level of functioning to implement effective treatment, and to plan appropriate discharge disposition. Conducts PASR screenings as needed.

·         Evaluates and maintains focus on patient safety at all levels of care.

·         Assesses substance use and history to plan appropriate level of care, interventions, and referrals.

o    Assessments demonstrating all baseline competency elements completed within required time frame

o    Documentation of psychosocial assessment completed

o    Re-assessment of patient conducted and documented as indicated

o    Families and significant others integrated in evaluation process with consent of patient as indicated

o    Patient strengths identified and documented

·         Maintains professional accountability.

·         Maintains and enhances social work skills.

·         Serves as patient advocate.

·         Complies with personnel policies, i.e. Attendance Policy, Dress Code, Social Work Licensure Practice Act, etc.

·         Recognizes and communicates ethical and legal concerns through established channels of communication.

·         Provides and accepts constructive feedback in a calm, respectful manner.

o    Licensure maintained

o    CPI, HIPAA, BLS/CPR, 8 hours intake training (if applicable), and continuing education required for license completed annually

o    Action taken to protect patients rights and/or preferences and promote patient desired outcomes

o    Patient advocacy role demonstrated and documented when appropriate and compliance evident

o    Ethical and legal concerns taken through appropriate levels of supervision

·         Participates as a treatment team member.

·         Coordinates patient care with other disciplines.

·         Builds professional relationships with peers, customers, and supervisors.

·         Offers assistance, preceptorship, and supervision to other staff and students.

·         Maintains rapport and confidence of physicians.

·         Adapts to changes in workload and shift assignments to meet changing needs of the patients or programs.

·         Maintains productivity.

·         Demonstrates positive, constructive communications.

o    Patient care consistent and coordinated between disciplines

o    Rapport and confidence of peers, customers, and supervisors maintained

o    Assistance and mentoring willingly provided to peers and students

o    Provides oversight for assigned students and monitors performance

o    Physicians express confidence and rapport with staff member

·         Provides discharge planning consistent with patients level of functioning and biopsychosocial needs.

·         Assists and educates patient and families with identifying appropriate transition plans.

·         Provides information on resources available based on identified patient needs.

·         Collaborates with treatment team and physician to establish and facilitate discharge plan.

·         Coordinates referrals and transfers with providers, community agencies, EAPs, payor source, and staff.

·         Obtains discharge plan orders as indicated.

·         Documents transition as appropriate for program.

o    Transition planning appropriate for each individual patient completed demonstrating baseline competency elements

o    Transition plan and resources documented

o    Appropriate referrals/appointments made based on identified patient needs

o    Knowledge of community resources demonstrated

o    Communication with treatment team and physician evident

·         Assists with the development and documentation of comprehensive treatment plans for each patient.

·         Identifies realistic, appropriate, and individualized goals for patients care.

·         Plans care to meet treatment plan goals.

·         Produces documentation that supports the treatment plan and meets regulatory requirements.

·         Coordinates care with treatment team and patient.

·         Orients patients to programs and unit guidelines.

o    Individualized treatment plan goals, initial assessment, and reviews documented

o    Goals are added and updated as appropriate to patient progress

o    Interventions are goal directed

o    Documentation is complete, accurate and legible

o    Attends and participates in assigned Treatment Team meetings

·         Participates in providing effective and cost efficient quality program services. Works to continuously improve quality of service programs. Maintains accurate and complete billing and accounting records. Understands and adheres to regulations and to accreditation standards. Maintains and enhances knowledge and skill in psychiatric and chemical dependency treatment.

o    Process improvement projects completed when asked

o    Key Performance Indicators and Key Initiatives acknowledged and actions taken to meet their requirements

o    Attendance or review minutes of all Interdisciplinary Staff, PASR and other required meetings documented

o    Appropriate utilization of resources exhibited

o    Expectations of customers including patients, families, EAPs, psychiatrists, PCPs, external case managers, referral sources, and receiving resources met or exceeded
