SUMMARY

Serves as an integral member of the assigned Mercy Health Partners primary and specialty outpatient clinical care teams within the role of mid-level provider. Responsible for the provision of mental health services, case management support, staff consultation and training, and community education and outreach. Focus of duties will vary depending on academic training, experience and needs of the clinic/patients.

QUALIFICATIONS

Education

Minimum: Masters in clinical social work or licensed professional counselors MA, LPC;MS, LPC

Credentials/Licensure

Minimum: Eligible for licensure by the Michigan Board of Social Work as a Limited Licensed Master’s Social Worker (LLMSW) or Licensed Master’s Social Worker (LMSW).

Preferred: Licensed by the Michigan Board of Social Work as a Licensed Master’s Social Worker (LMSW), designated in clinical practice.

Related Experience

Minimum: Completed post graduate supervision and full licensure with applicable experience in medical social work, counseling, case management.

Preferred: One year of experience in medical social work, counseling, general social work.

Other Knowledge, Skills and Abilities

Minimum: Effective psychotherapy skills; strong report writing/documentation skills; ability to manage a variety of tasks and problems at the same time; high level of initiative and ability to self direct to solve problems; ability to work collaboratively with peers in a similar role

Preferred: Strong knowledge of community resources for patients and their families

Computer Competency

Familiarity with standard desktop and windows based computer system, including email, e-learning, intranet and computer navigation. Ability to use other software required to perform essential functions.

Physical/Mental

Minimum: Ability to work with a high level of activity, distractions, and numerous interruptions. Multiple assignments often need to be prioritized daily. Sitting for long periods of time. Ability to walk requests/reports around to other locations in or near the hospital complex. Ability to bend, stoop and reach above head, lift, and carry. Requires fine motor skills. Extensive use of keyboard, repetitive tasks of typing and phone use. Long periods of listening to phone calls. Rare occasion of lifting less than 20 pounds. Ability to coordinate work so deadlines are met (very tight deadlines on many assignments). Requires flexibility in work hours. Ability to troubleshoot variety of problems. Ability to lift up to a maximum of 35lbs. and perform push/pull tasks requiring a maximum force of 35 lbs. Ability to physically operate safe patient movement equipment in compliance with the Safe Patient Movement Policy.

ESSENTIAL FUNCTIONS

 Provides triage, comprehensive biopsychosocial and psychiatric assessment, psychotherapy, consultation and care coordination support.

 Provides individual, family and group therapy; short-term as well as traditional psychotherapy.

 Provide clients with self-management skills and educational information so they can be full participants in their own treatment and recovery.

 Monitors patient progress for achievement of health goals and outcomes.

 Maintains strong, working relationships with other members of the treatment team. Participates in case consultation and department meetings.

 Provides timely feedback to providers about the client's care, treatment recommendations, and progress via verbal communication and current EHR documentation.

 Provides consultation and training to Mercy Health Partner providers and staff.

 Monitor and coordinate the delivery of health services including linking with other treatment providers as appropriate.

 Coordinates referrals to specialty care and community services.

 Provides outreach and community education.

 Maintains competency and enhances professional growth and development through participation in educational programs, current literature reviews, case consultations and professional conferences.

 Performs other related duties as assigned.
