CHI Health has an immediate opportunity for a Behavioral Health Transfer Care Coordinator 

CHI Health Midwest’s Behavioral Health Evaluation and Transfer Service (BHETS) is a transformative model of care that will make behavioral health services accessible patients in the region. BHETS is an extension of the CHI Health Transfer Center service and virtual services designed to improve patient outcomes through synchronous tele-behavioral health services for patients presenting with mental and behavioral health conditions. The transitional care coordinator works closely with assigned physician(s) and the community to coordinate services, ensure continuity of care and reduce inappropriate hospitalizations. This position serves as a patient advocate and liaison to assist patients and families by providing support, education and following up to ensure patients are utilizing appropriate community resources in a timely fashion. BHETS is a 24/7 service. This role serves as the first point of contact for emergency departments, healthcare facilities, and community-based physicians/providers to coordinate emergent transfers and acute admissions to CHI Health tertiary hospitals

Responsibilities

Expectations:

Provide the physician and the health care team with recommendations and information which will assist in developing an overall plan for the patient’s care upon consultation.Partner with providers and hospital staff in effective discharge planning. Educates patients and families by providing facts about diagnosis, treatment plans and available community resources.Collaborates with the physician(s) and health care team and serves as a patient advocate and liaison with community agencies. Develops and maintains good working relationships with community treatment network to ensure the availability of quality services. Coordinates care with other health care community providers. Connects patients and families with these providers/services.
Qualifications

Minimum Qualifications:

Bachelor’s degree in Social Work or Human Services related field, preferredAcute care experience as a social worker preferred
