Bereavement Coordinator needed for St. Mary’s Home care & Hospice in the Athens area
Basic Job Function Summary
Provides professional bereavement care to Hospice client families and significant others after the client’s death via counseling, education and coordination of services in accordance with prescribed Hospice standards and agency policy and procedure. Uses problem-solving skills in assisting with Plan of Care relative to the grief process.
Minimum Qualifications
• Baccalaureate degree in social work, psychology or counseling required.• Master’s degree with major course work in social work, psychology or counseling preferred.• Two years of Hospice clinical grief counseling experience or an equivalent combination of education and experience required.• Current working knowledge of conditions of participation and hospice licensure regulations.• Clinical experience of skills relevant to grief and/or loss required.• Must have current Driver’s license and reliable transportation to and from work site.Essential Functions
• Collaborates with IDT/IDG members to address grief/bereavement needs of the patient/family and participates in the ongoing updates to the plan of care. As well as potential for anticipatory grief and complicated grief.• Effectively and efficiently prioritizes visits and services based on survivor assessments and the bereavement plan of care.• Provides bereavement assessments, determines survivor grief needs and develops the bereavement plan to meet those needs.• Makes appropriate referrals, internal and external to the agency, to assist survivors as appropriate.• Coordinates and evaluates the implementation of the bereavement care plan for 13 months after the death of the client in accordance with regulatory, accreditation and coverage requirements.• Works with scheduling to assure bereavement care services are scheduled in accordance with the plan, maintains an appropriate productivity level and is suitability assigned to other team members when needed.• Actively participates in marketing/community education and business development activities as assigned by supervisor.Benefits
