The Bereavement Coordinator I is a member of the Providence TrinityCare Hospice Bereavement Team. The Bereavement Coordinator I provides bereavement services to low risk bereaved family members of hospice patients. These services include assessments, treatment plans, information and resources, grief education, crisis intervention, bereavement counseling and support group facilitation. Outreach and education are also provided to the community. The Coordinator may provide care to premature infants, infants, pediatrics, adolescents, young, middle and older adults.

Required qualifications:
Bachelor's Degree in Social Work or in related field Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information. 3 years experience in the area of bereavementPreferred qualifications:
