**Bereavement Counselor/Social Worker**
Hospice - Albany
Per Diem - Temporary-Grant Funded
The Community Hospice has a Per Diem opportunity for a Bereavement Counselor/Social Worker to join our hospice care team caring for patients in the Capital Region area as part of our Interdisciplinary Team caring for patients and families receiving hospice care. As a member of the hospice interdisciplinary team, the primary focus is to respond to the emotional and psychological needs of patients, families and staff.
**What you will do:**
This is a temporary, grant-funded position. The Bereavement Counselor is responsible for working with the bereavement team to coordinate bereavement services and to provide direct counseling services to individuals and groups. Bereavement staff connect with the primary care givers of patients on The Community Hospice program and connect with them for 13 months post death through mailings, phone calls, memorial services and direct counseling services. The Bereavement counselor provides group and individual grief support to The Community Hospice staff. The Bereavement Counselor will provide grief related education, trainings, and will facilitate group support upon request.
**Responsibilities:**
+ Provide direct bereavement counselingservices in individual and group format to people of all ages in rural communities within the 8-county service area of The Community Hospice.
+ Initiates and pursues a bereavement care plan according to the Bereavement Services Policy, reflecting family needs and frequency of services. Honors family and caregiver’s choices regarding bereavement contact or services.
+ Evaluates and assesses grief and loss issues for primary care givers of Hospice patients, families and community members as requested.Will remain available to the family during the 13 month bereavement period.
+ Provides bereavement counseling to residents of skilled nursing facilities, adult homes and assisted living programs, upon request.
+ Identifies clients whose needs require additional professional involvement and refers appropriately.
+ Provides bereavement counseling to pediatric clients both individually and via our Wave Riders support groups in partner school districts within our service area.
+ Will provide educational programming regarding advance care planning, end of life care, grief, and The Community Hospice as requested.Will include goals, objectives, and evaluation of educational activities.
+ Serves as a resource to community agencies and organizations to plan for and respond to death related crises and disasters.
**What you will need:**
+ Bachelor's Degree in Human Services field is Required.
+ Master's Degree in Social Work, Counseling or related field from an accredited college or university.
+ NYS License Preferred (LMSW, LCSW, LMHC)
**Pay Range:** $25.85 - $36.67
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
