Part-Time - 9am-5pm - 3 days a week (E
The Community Hospice has a part time opportunity for a Bereavement Service Facilitator to join our hospice care team caring for patients in the Capital Region area as part of our Interdisciplinary Team caring for patients and families receiving hospice care. As a member of the hospice interdisciplinary team, the primary focus is to respond to the emotional and psychological needs of patients, families and staff.
**What you will do:**
The Bereavement Services Facilitator will assist the bereavement team in the operation of the bereavement program by: Completing intakes with new clients and providing referrals when appropriate. Making follow-up calls to assess bereavement needs of primary care person and families; co-facilitating in-services and support groups; organizing regular bereavement mailings; organizing regular memorial services. Supports bereavement program by attending team meetings, attending site and discipline specific staff meetings.
**Responsibilities:**
+ Completes initial intakes with new clients and provides referrals when appropriate.
+ Coordinates school-based grief support with local school districts to organize individual and group programming as requested.
+ Makes follow-up calls to assess bereavement needs of primary care persons and families.
+ Coordinates and participates in in-services, support groups, and on-site family groups
+ Provides individual and group grief support to families, agencies, Hospice staff, volunteers, interns and medical students.
+ Provides community education activities and include goals, objectives and evaluation of educational activities.
+ Honors family and caregiver’s choices regarding bereavement contact or services.
+ Organizes and facilitates memorial services on-site and in the community
+ Assists with coordination and participation in crisis responses.
+ Assist with planning for annual Camp Erin and attendance at annual overnight camp is required
+ Participates in collection and tabulation of bereavement program statistics, utilizing queries to invite specific populations to groups and services.
+ Inputs intakes and creates patient charts in electronic medical record in a timely manner.
+ Maintains and updates electronic bereavement files. Reviews patient charts to ensure all data is complete and accurate.
+ Participates in regional and agency-wide QI initiatives related to the Bereavement process/program.
+ Participates in orienting staff, volunteers, interns and medical students to bereavement services.
+ Attends Hospice team and staff meetings.
+ Answers and screens phone calls on a daily basis.
+ Schedules in-services and bereavement groups/meetings as needed.
+ Maintains confidentiality of patient information except when patient has given permission for disclosure. Maintains confidentiality of personnel information.
+ Meets individualized productivity expectation. Promotes and utilizes performance improvement process in daily work routine and contributes positively to the performance improvement process (including participation on committees) within the Agency and St. Peter’s Health Partners.
+ Participates in staff development programs, in-services, continuing education opportunities and team meetings to keep current and proficient in job responsibilities. Responsible to attend mandatory in-services and to understand your role in the organization’s corporate compliance plan.
+ Maintains professional knowledge and skill level appropriate to services being provided and up-to-date knowledge of Agency policies and procedures. Applies this knowledge in everyday practice.
**What you will need:**
+ Associate Degree in Human Services field is required.
+ Bachelor's Degree in Human Services field is preferred.
+ 2 Years experience with clients in a human service capacity.
**Pay Range:** $19.20 - $27.99
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
